The UVA Department of Psychiatry and Neurobehavioral Sciences seeks candidates for a Patient Navigator. This individual will be responsible for assisting patients by providing hands-on and diligent support to encourage navigation of systems, adherence to individualized treatment plans, and promote overall engagement in the process of navigating their healthcare needs. This individual will act in an essential role supporting and guiding patients as they address their substance use and co-occurring mental health and medical conditions.
Duties may include:
Provide ongoing support and assistance to patients as they navigate their treatment plan with the interdisciplinary team.Encourages patients to be proactive regarding their recommended treatment plan.Communicate with the patient and pertinent stakeholders regarding patient status, status of referrals, and other outreach or follow-up efforts.Provide care coordination and resource navigation services using a patient- and family-centered approach.Assist patients and families in learning to communicate effectively with healthcare and other service providers.Assist patients and families in identifying and prioritizing their needs.Identify and address service gaps and barriers that make it difficult for patients and families to meet their healthcare needs.Collect and distribute resource materials to foster understanding of patient care needs.Develop and maintain ongoing connections with community resources, support groups, and other service providers.Facilitate patient enrollment and participation in any research or other relevant healthcare initiatives, as appropriate.Investigate, document, and resolve or refer, as appropriate, problems and conflicts to social work manager or other applicable healthcare staff.Evaluate processes and makes suggestions for improvement, as needed.Assist the team to address specific research and clinic needs, as indicated.Assist with project development and expansion of programs through planning and coordination of services.Provide support for helpline calls.In addition to the above job responsibilities, other duties may be assigned.The Patient Navigator will also assist with problem solving of patient care issues and facilitate communication with the patient’s care team, as necessary. The incumbent must have a positive “customer-focused” attitude, be passionate about assisting patients, and enjoy contributing significantly to the research and clinical teams in support of various project goals.
MINIMUM REQUIREMENTS
Education: Bachelor’s Degree or equivalent combination of experience, education, licensure/certifications may be accepted in lieu of degree.
Experience: One year of experience working with complex patient populations.
PHYSICAL DEMANDS
Job requires frequent bending/stooping/walking. Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious disease.
