Patient Navigator
Desert Parkway Behavioral Healthcare Hospital
The Patient Navigator is responsible for patient coordination of care to outpatient services. Communicates with all disciplines to facilitate seamless continuity of care and recognizes priorities of tasks and responds accordingly.
Outpatient Service Programming Support
+ Integrates and coordinates with the Inpatient treatment team: attends meetings related to treatment and discharge planning, working with case managers, therapists and physicians.
+ Works with case managers regarding those patients that have not been referred to Outpatient to overcome barriers and issues preventing referral to Outpatient services.
+ Meets with appropriate patients to educate them about the services offered at Outpatient and explains the continuum of care.
+ Facilitates successful inpatient step-downs to Outpatient services.
+ Meets with parents of appropriate adolescent patients regarding Outpatient services.
+ Tracks, trends, and collects data and produce management reports for areas related to Inpatient step-downs and transitions.
+ Completes all clinical documentation in a timely manner and according to established standards.
+ Conducts 1:1 visit with individuals identified as potential participants in DPBHH Outpatient programming. Reports visit outcome as required.
+ Reaches out to potential patients who have inquired about outpatient programs through the website.
+ Coordinates/collaborates with Intake Department on assessed patients not meeting inpatient criteria and referred to outpatient.
+ Facilitates direct admission to appropriate program for patients referred by behavioral health provider.
+ Coordinates/collaborates with outpatient program lead and reception to provide seamless transition from inpatient to outpatient treatment.
Case Management/Social Services Oversight and Direct Support
+ Establishes relationships and communicates regularly with all disciplines that are working with the patient and the patient’s family/guardian.
+ Provides clinical information to Utilization Review staff.
+ Problem solves cases with physicians and clinicians in an effort to plan, monitor, and provide treatment that supports progress toward established treatment goals.
+ Contributes to provision of continuity of care for patients as they move through the clinical continuum.
Requirements
+ Master’s degree in Social Work or related field required.
+ Experience providing group/individual/family therapy, conducting clinical interviews, and coordinating/contributing to case management/social work services.
+ Experience in healthcare and business development/communication environment preferred.
