Position Summary:

Patient Navigators provide care coordination to future, current, and past patients referred to the Maternal Addiction Resource Center.  Facilitates engagement and re-engagement in substance use disorder treatment and aids in coordinating appointments, services, additional medical, pregnancy care, behavioral, and social needs as defined by the program. Works with community entities/services to navigate patients to services for safe personal, infant and child care pre-delivery and up to 1 year post-delivery.

Minimum Qualifications: High School Diploma or equivalent.  Minimum of two-year degree in Health Care or Social Work related field, from a college/university (i.e., RN, BSN or LSW) Minimum of 2 years of experience with pregnant patient population or drug & alcohol treatment, and community networking. License as required.
