Job Overview:
TRC Patient Navigators are expected to develop relationships based on trust with patients and families utilizing a trauma-informed approach, provide reliable information, appropriate community resource referrals, and emotional support to victims of violence. The TRC Patient Navigator will meet with victims in the community, as well as, structured and assertive outreach meetings to referral resources and community supports.. This individual will function as a core member of the TRC treatment team.
Qualifications:
Required:
Bachelor s degree required, BA in counseling, social work, psychology, forensics or related field preferred. Successful completion of training including hospital-wide orientation, trauma informed care training, orientation to the Trauma Recovery Center mission and service, and shadowing of community outreach organizations required. Two (2) years of recent experience, such as any combination of student, volunteer, or work experience, which involves the Jersey City area and its community resources, is required. NJ active driver's license required.Must have a basic understanding of computer use and software applications, and the ability to perform basic internet searches. Must have demonstrated interest in serving the Jersey City community. Must be able to work independently and seek guidance when necessary. Must have good listening skills. Must have a demonstrated ability to work in a team environment and have the ability to work with individuals of a diverse background.Must have own transportation to accompany patients to appointments and connect with community resources, as needed.Preferred:
Participation in community organizations and firsthand understanding of issues related to interpersonal violence preferred. Sensitivity to a substance abuse environment preferred. Previous mentoring experience is desired. Bi-lingual in Spanish strongly preferred.Benefits and Perks:
