If so, Callen-Lorde wants to hear from you! We are seeking a Patient Navigator to join our dedicated team and play a vital role in supporting the needs of our patients living with HIV. As a Patient Navigator, your responsibilities will include: + Act as a patient advocate, ensuring patients' biopsychosocial needs are identified, addressed, and connected to appropriate services. + Provide field-based case management and adherence support services to HIV-positive patients. + Work collaboratively with care coordinators, medical providers, and behavioral health professionals to facilitate patient retention and adherence to primary medical care. + Assist patients in navigating the healthcare system, overcoming barriers to care, and connecting them with essential resources. + Advocate for patients' rights and ensure they receive culturally competent care. + Document patient interactions and progress notes to ensure continuity of care. + Contribute to a team-oriented environment, fostering a supportive and inclusive space for patients. Work Schedule: This is a full-time position with evening hours. Qualifications + College degree (BA/BS) in health/social services or related field required. + Experience in social services, case management and/or entitlements work. + Familiarity with LGBTQIA, HIV and health-related issues and community resources. + Strong computer competency, including knowledge of Electronic Medical Records, Microsoft Office Programs, and internet research savvy. + Strong customer service orientation with an understanding of patient-led treatment. + Experience working with individuals with a spectrum of chronic illness conditions preferred. + Bilingual (Spanish/English) preferred.
