The University of Miami Health System, "UHealth", Department of Pediatrics- has an exciting opportunity for a Full-Time Patient Navigator/Community Health Worker to work in Miami.

The Patient Navigator/ Community Health Worker facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

Core Responsibilities:                                                                                                   

Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.

Ensures patients have access to insurance and social service programs for which they are eligible.

Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.

Identifies and resolves real or perceived barriers to care.

Educates and counsels patients on issues related to their health and life situations.

Maintains current knowledge of community health, welfare resources, and any other programs available to patients.

Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions                               

The Patient Navigator/Community Health Worker will be knowledgeable about community resources to assist families of children and young adults impacted by sickle cell disease.

Work collaboratively with another community health worker, a social worker and other members of the pediatric sickle cell program.

Meet with families in order to inform about available resources. Sometimes the patient navigator may visit patients in the hospital.

Occasionally, the patient navigator will have to visit patients’ homes, such as in the case of patients who are difficult to reach.

Knowledge of other languages (Haitian Creole or Spanish) besides English is optional but recommended.

Specific sickle cell community health worker training may be provided depending on availability.

CORE QUALIFICATIONS:                                                                                                    

High School diploma or equivalent

Minimum 3 years of relevant work experience

Bilingual (Haitian Creole or Spanish) preferred.

Ability to process and handle confidential information with discretion.

Ability to communicate effectively in both oral and written form.

Ability to handle difficult and stressful situations with professional composure.

Ability to maintain effective interpersonal relationships.

Ability to understand and follow instructions.

Any appropriate combination of relevant education, experience and/or certifications may be considered.
