MAJOR FUNCTION: 

The Patient Navigator will work with individuals to support and strengthen their capacity to achieve and maintain their mental health and engage in healthy practices.

QUALIFICATIONS: 

Education & Experience: 

Bachelors degree in Social work, psychology, or other behavioral health required.  Three years' experience working with behavioral health and/or autism field required.  Experience working with children, adolescents and adults preferred.

Certifications/ Licensure: 

BLS and Welle competency upon hire preferred, must be obtained during introductory period.  Physical Requirements:
