Patient Navigator LPN
**The Patient Navigator, LPN Monday-Friday 8:00 AM-4:00 PM** is responsible for working in coordination with care management to assist inpatient and ED patients requiring transport from Lower Keys Medical Center to other hospitals and will support the patient and family members in understanding the care and transport plans. The patient navigator will help navigate and coordinate needed resources such as transportation and temporary housing, supporting the patients return to the Key West community, and assisting when possible with ongoing medical support plans once back in the keys. **Requirements:** + An Associate's Degree and LPN (FL) certified are required. + 2+ years of LPN experience in a hospital setting + Current BLS and PALS certification from the American Heart Association. + Familiarity with a variety of the field's concepts, practices, and procedures. **About the job:** + Currently working knowledge of discharge planning, utilization management, care management and disease management + Understanding of pre-acute and post-acute venues of care and post-acute community, resources, physician office routines and transitional procedures for pre-and post-acute care + Knowledge of Medicare preferred + Demonstrated ability to communicate effectively in person and via telephone with patients, families/caregivers, physicians, physician office staff and Post-Acute providers using appropriate dialogue and customer service competencies.
