Serve as the Hematology-Oncology Community Clinics Patient Navigator Manager providing non-clinical support, guidance and assistance for patients and families living with cancer.  Primary goal to assist the patient with overcoming health care barriers and act as a liaison to understand patient’s non-clinical individual needs and concerns.  Guides the patient and family to a broad range of services and resources across the oncology care continuum and works as a part of a multidisciplinary team to optimize care and outcomes.

Salary range: Salary range: $68,800/year – $141,000/year
