Job Overview:

The primary purpose of the YHK (Your Health Kiosk) Patient Navigator is to serve as a role model and community liaison to individuals who are enrolled in the Your Health Kiosk Program. Patient Navigators are expected to build effective working relationships with affiliated and non-affiliated physicians, senior centers, hospital providers, family members/care givers and community organizations to improve individualized patient care coordination and reduce barriers across the health care continuum. They are expected to develop relationships based on trust with patients and families, provide reliable information to appropriate community resource referrals, coordinate physician office visits, outpatient treatment, surgical and specialist referral appointments, transportation arrangement, and accompany patients to offsite physician offices and/or hospital departments to provide language

Qualifications:

Required:

Bachelor s degree required or equivalent experience in related fieldTwo years of recent experience, such as any combination of student, volunteer, or work experience in care coordination and referral to community organizationsMust have a basic understanding of computer use and software applications, and the ability to perform basic Internet searchesMust have strong written and oral communication skillsMust have strong organization skillsMust be able to work independently and seek guidance when necessaryMust have good listening skillsMust have a demonstrated ability to work in a team environment and have the ability to work with individuals of a diverse backgroundMust have own transportation to accompany patients to appointments and connect with community resources, as needed

Essential Functions:

Actively contributes to creating a friendly, welcoming and safe environment for our community membersCommunicates clearly to ensure understandingResponds to complaints and concerns by resolving issues immediately whenever possible.Organizes work process to accommodate community needs, including internal customers and coworkers.Meets or exceeds community expectations for serviceActs as a role model for service excellence.Maintains professional composure in the work placeWorks effectively with the team to ensure customer service scores for work are meet/exceed targets.Demonstrates respect for patient rightsCoordinate appointments for individuals enrolled in program to outpatient departments, providers and private physicians to ensure timely delivery of diagnostic, treatment services and specialist referralsHelps community members access services by assisting in scheduling appointments, reminding them of appointments, and accompanying to appointments when necessaryEvaluates and assesses the community members current community-based needs and barriers to meeting those needs. Collaborates with community organizations to provide outreach servicesCompletes program intake assessments, consents, and documents intake measures in the designated locationFacilitate communication between the patient, family members, and healthcare providers to ensure the highest patient satisfaction and quality of careProvides community members enrolled in program with referrals, encouragement, and resources to address the needs identified by the Patient Navigator and the YHK teamAssists in the development of YHK program with demonstrated outcomes in care coordination and connection to community resourcesMeet with community member enrolled in program when necessary, to ensure recommended services are being received. Actively supports and monitors client linkages with servicesAs part of a multidisciplinary team, provides feedback to the YHK team regarding individual program and patient needs and concernsManages, documents, and maintains productivity log on an ongoing basis as requiredPeriodically reassesses participants needs, effectiveness of interventions, and attainment of chosen outcomesAttends community outreach events to present YHK and enroll community members into the program as neededAttends educational trainings offered by the hospital and communityFacilitates relationships and documents interactions with community-based organizationCompletes regularly scheduled computerized data entry for program monitoringMaintains confidential files and current records on clients, providing data to the director when needed
