Job Overview:

The Patient Navigator will provide a warm hand-off to providers, escort patients to appointments, provide patient education, and maintain contact with patients throughout their care. In addition, the Patient Navigator will coordinate internal and external referrals for prevention, support and wellness services, specifically integrated screenings and provide continuous support for all clients who have been tested for HIV regardless of test result.

Qualifications:

Required:

Associate Degree required

Preferred:

Bi-lingual, and able to read, write and speak Spanish proficiently, highly preferredExperience in a healthcare setting is highly desirableProficient in Microsoft Word, Excel and PowerPoint

Scheduling Requirements:

Monday Friday 9:00am-5:00pm Require some weekends and attend Community Health Events that occur

Essential Functions:

Advocate for clients who have legal benefit concernsAssess client needs, prepare appropriate individual service plans and maintain recordsAssist clients in implementing the service plan by networking and referral, internally and/or to other community resourcesAssist homeless or precariously-housed clients towards more stable housingAssist with insurance and benefit applicationsAttend and participate in staff meetings and supervisory conferencesConduct referral and linkage activities to housing, food programs and other customer needsCoordinate with social services and insurance companies or other financial services to ensure the ability to pay for careDevelop and maintain a database of professional contactsEnsure confidentiality of all recordsEstablish, maintain and update client case records using approved agency forms and proceduresMaintain and update client information in a specific databaseMaintain working relationships with other community providersPrepare monthly reports in accordance with agency policies and proceduresProvide relevant literature to clients and members of the communityProvide written reports and maintain statistics as requiredRepresent the agency at community events, meetings, and conferences
