Hourly Pay Range (Minimum - Midpoint - Maximum):

$27.280 - $34.100 - $40.920


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do
The Patient Navigator-Oncology (Non-Clinical) role is to provide individualized care to oncology patients, their families, and caregivers with the primary goal of overcoming barriers within the healthcare system and facilitating access to all resources available to patients. Through initial and ongoing patient needs assessments, the oncology patient navigator will pair each patient and family with the appropriate internal and community resources that will streamline their cancer care journey and ensure excellent care across the continuum. The oncology patient navigator will serve as a link between patients and clinicians to facilitate supportive care services and will improve quality and timeliness of care, thereby increasing adherence to recommended treatment plans and ensuring the best care outcomes for patients and their families. The role is a patient-facing point of contact to empower patients in dealing with their disease process and treatment.

Required QualificationsH.S. Diploma or Equivalent 2 Years Patient-facing role in healthcare or healthcare administration.
Preferred QualificationsAssociate's Degree in a healthcare related field. Experience working with oncology patients and families.
Other Qualification RequirementsOncology Patient Navigator- Certified Generalist (OPN-CG) strongly preferred.
Essential Functions
Internal and External Communication
Collaborate with interprofessional team members across departments caring for oncology patients, including providers, registered nurses, radiology team members, social services, registered dieticians, radiation therapists, etc. and develop streamlined referral processes.
Establish excellent communication with patients, families, and the interprofessional team.
Appropriately represent Sharp HealthCare at all times when in contact with patients, families, and community members.Internal and External (Community) Resources
Conducts basic health literacy assessments to ensure patient and family are receiving information through their preferred method.
Conducts initial and ongoing patient needs assessment to determine the internal and/or community resources that would best serve each individual patient.
Initiate referrals to internal and community resources as appropriate, including referrals for lodging assistance, transportation assistance, and food insecurity assistance.
Appropriately assign and escalate requests for clinical support to the interprofessional team (clinical counselors, registered dieticians, registered nurses, etc.)
Encourage patient and family participation in decision making process.
Protects patient privacy by maintaining confidentiality and abiding by HIPAA regulations.Patient Advocate
Demonstrates excellent communication skills with patients and families.
Listens to concerns from patients and/or families and escalate to the appropriate part for resolution.
Serves as a liaison between healthcare professionals, patients and families.Teamwork and Communication
Consults with team members in development of referrals and resource recommendations.
Maintains on-going communication with interprofessional team members regarding patient status and follow-up.
Documents patient interactions, recommendations, and interventions appropriately, ensuring communication to all involved care team members.Other Duties
Maintain knowledge of the standards and requirements of the accreditation body.
Track patient referrals and data related to patient care.
Assist with developing reports regarding patient access and actively participate in improvements to program or internal processes.
Maintain appropriate inventory of patient education materials, new patient packets, etc.
Other duties as assigned.
Knowledge, Skills, and AbilitiesAbility to be innovative, creative, and resourceful in order to develop strategies that successfully meet the needs of diverse patient, family, and provider populations. Demonstrate competence in team building, conflict management and interpersonal effectiveness. Strong communication and interpersonal skills in building relationships with physicians and other members of the healthcare team. Excellent computer application and database management skills. Excellent written and spoken communication skills required.
