Make a Difference
The Patient Navigator offers individualized assistance to patients, families and caregivers to help overcome health care system barriers and facilitate timely access to quality medical and psychosocial care. They collaborate with other members of the care team to provide consistent access to exceptional care delivery. The navigator will participate in development of the patients’ plan of care and implementation of the plan.
Exceptional Skills and Qualifications
Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.
2 years previous healthcare experience, specifically in Pulmonary or Radiology required Bachelor degree or equivalent years of department specific experience Exhibit excellent interpersonal skills to provide patient support Work closely with RN navigator to ensure patient has complete understanding of their diagnosis and plan of care Demonstrate a commitment to serve as a patient advocate. Demonstrate the ability to work independently or as a team member to meet time driven deadlines. Demonstrate the ability to communicate in a clear and concise manner. Working knowledge of oncology diagnoses and patient flow within the oncology department. Medical office experience preferred EPIC EMR system and scheduling experience preferredCertified Medical Assistants, Social Workers (BSW), Licensed Practical Nurses, or Certified Community Health Workers also eligible with 2+ years of Pulmonology experience
