This is a full time, day shift Patient Navigator (RN) position supporting Northwest Medical Center.
The Patient Navigator serves as the liaison between the clinic, medical providers and the hospital center. The Patient Navigator will have primary responsibility for patient education, staff education in the clinic and evaluations for patients in the process of preparing for surgeries, procedures or other specialized medical care. In addition, the Patient Navigator is responsible for maintaining relevant policies and procedures, outcome data collection and quality improvement efforts.
Position Qualifications/Minimum Requirements
Education: Required: Associates degree or diploma in nursing program
Preferred: Bachelors degree in nursing
Experience: Preferred: Three (3) years experience with clinical specialty patient population.
Skills: Compile statistics; computer literacy, data entry, retrieval, reporting; input data into computer programs; interview others; research information; strong analytical and organizational skills; and team facilitation and meeting skills. Excellent human relations and oral/written communications skills. Computer skills including word, excel, and graphic.
Licenses/Certifications: Required: Current AZ or compact state RN license
