Patient Referral Intake Navigator
Overview This position is remote working Thursday- Monday 8:30-5:00. Responsibilities As a Referral Intake Navigator, you will play a pivotal role in shaping the efficiency and success of our centers by expertly managing the intake process. Your primary responsibilities will include conducting thorough clinical and financial reviews of referrals, verifying insurance benefits, and handling prior authorizations. You will make critical decisions by approving or escalating referrals based on each facility's clinical and financial capabilities. This role involves extensive collaboration with a diverse range of stakeholders, including hospital systems, case managers, and facility leadership teams. In addition to responding to inquiries from hospital discharge planners, families, and other referral sources, you will leverage our advanced e-referral systems and CRM tools to streamline the intake process. By utilizing data and analytics, you will ensure rapid responses and optimize referral management, enhancing care transitions and maintaining high standards of patient care. Your contributions will directly impact the coordination and business development outcomes across our centers, driving excellence in community service and patient care. In this role, you will ensure that potential admissions are seamlessly prepared for transition by effectively communicating admission and transitional care needs with the relevant staff members. Success in this position requires a thorough understanding of Federal and State regulations, referral processes, and reimbursement mechanisms, including Managed Care, Medicare, and Medicaid. Your ability to stay informed and navigate these areas will be crucial in facilitating smooth transitions and maintaining high standards of care. Key Responsibilities: Referral Generation & Management: + Utilize established relationships with case managers to generate a consistent stream of referrals for the centers(s). + Identify and build on new relationships or new positions within the hospital system to drive more referrals to the assigned center(s). + Own the referral process from start to finish, ensuring that all referrals are processed efficiently and effectively. + Demonstrate proficiency in the clinical review process to assess patient needs and match them to the appropriate center. Collaborative Relationship Building: + Collaborate closely with the center and market clinical team to ensure a seamless pathway to admission for referred patients, emphasizing clinical excellence and patient satisfaction. + Maintain effective communication with patients and their families throughout the referral and admission process through phone calls, emails and documentation in our CRM PCC system. + Establish strong working relationships with key personnel within the center(s), including Directors of Nursing and the Internal Admissions team. + Have a deep understanding of the capabilities and differentiators of their assigned center(s) and effectively communicate these to potential partners. Strategic Business Development: + Although not responsible for new sales, strategically build alliances with the referrals received and identify partnership opportunities with the hospital system to assist with patient throughput. + Identify pathways for partnership within the hospital system and facilitate or schedule meetings to explore collaboration opportunities. Qualifications + Minimum of two (2) years’ experience in skilled nursing, long term care, or other related environment. Direct admission experience and referral management experience preferred. + Strong understanding of the long-term care business, including census and quality mix goals and referral expectations. + Ability to navigate multiple electronic healthcare referral systems. + Proven track record of success in driving business growth, achieving sales targets, and leading high-performing teams. + Deep understanding of healthcare regulations, reimbursement models, and industry trends impacting skilled nursing and rehabilitation facilities. + Excellent communication, negotiation, and interpersonal skills, with the ability to collaborate effectively across all levels of the organization. + Strong analytical skills, with the ability to interpret data, analyze trends, and develop actionable insights. + Proficiency in Microsoft Office Suite and CRM software. + Willingness to travel nationally as needed. Posted Salary Range USD $80,000.00 - USD $85,000.00 /Yr.
