GENERAL SUMMARY AND PURPOSE:

Elevate your professional practice and career by applying your clinical, communication and educational skills by becoming a Patient Safety Coordinator.

Under the supervision of the West Region Director of Risk Management and Safety, evaluates and implements the aspects of the organization’s overall plan for patient safety that involves any aspects of patient risk, both within the ministry as well as the clinic setting. Focuses on patient safety priorities as defined by the West Region Director of Risk Management and Safety.

Provides facilitation and consultation to the Nursing Unit Champions and is an active participant in the Nursing Council structure. The Patient Safety Coordinator communicates regularly with the board (governance) and senior administrative leadership; the position is a primary point of contact of the integrated patient safety program.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

Must be licensed as a Registered Nurse. Bachelor's degree in nursing required.

Experience as a program coordinator, in a nursing leadership position or as an educator preferred. Basic database development, data display and analysis preferred. Knowledge of evidence-based practice methodology preferred.

ESSENTIAL FUNCTIONS:

Participates in the development of the organization’s risk and patient safety strategic plan as it pertains to safe nursing practice. Develops and provides education and mentoring to Unit Champions and/or SAMG nurse leaders

Participates in Unit Champion meetings, and other committees/meetings that align with patient safety initiatives. Assists Quality with LeapFrog compliance Makes frequent rounds on the nursing units/clinics to: Consult with nursing, Radiology Technologists and Unit Champions to identify risk and safety issues. Observe and evaluate practice/process change progress. Provide just-in-time training and answer questions.

Conducts audits or other data collection methods to evaluate progress on outcome or process measures. Maintains databases on process and outcome metrics. Displays, analyzes, and communicates findings related to nursing quality and patient safety. Maintains compliance with Joint Commission standards. Identifies evidence-based practice related to patient safety initiatives and assimilates evidence-based practice into current processes. Self-directed and professional with critical thinking skills. Exercises judgment and tactful discussion of clinical quality and safety issues.

Participates in the implementation of corrective action plans resulting from the CERT’s, ACA’s RCA’s process, including Common Cause Analysis (CCA) as needed.
