The Patient Safety Coordinator is responsible for collecting, transforming, analyzing and reporting quality/patient safety outcomes and process data that support stakeholder's decisions related to quality initiatives, patient safety, performance improvement initiatives, regulatory standards compliance, and policies and procedures. The coordinator will be responsible for preparing, analyzing and distributing reports in the form of dashboards, scorecards or other presentation tools, ensuring the accuracy, integrity, validity, reliability and consistency in report preparation
JOB SUMMARY
Ability to communicate effectively verbally and in writing.Must be able to work in a union environment. Minimum of one (1) year analytic/statistical analysis experience, preferably in health-care or clinic environment.Knowledge of the factors contributing to quality patient care, and the ability to influence these factors in a positive way.Knowledge of quality management methods, tools, and techniques and ability to create and support an environment that meets the quality goals of the organization.Knowledge of the critical interdependencies among system elements that help and hinder performance and ability to plan and design solutions that synchronize resources to achieve business results.Knowledge of federal, state and local healthcare related laws and regulations; ability to comply with these in healthcare practices and activities.Knowledge of the risks faced by patients in a healthcare environment, and the ability to take steps to reduce these risks.Facilitates development and implementation of data collection tools and processes including, the ability to identify data elements needed to complete appropriate measurement, perform timely data collection and abstraction per specifications, and validate data prior to submission or preview reports prior to publication.Generates reports and analyses to support related quality improvement, patient safety, patient experience and operational effectiveness using key statistical tools, including but not limited to, run charts, bar charts, pie charts, control charts, and cause and effect diagrams. Analyzes data and identifies key findings and develops action plans. Presents complex data with simplicity of design in a clear, easy to understand format.Learns, understands and becomes proficient in relevant facility, HPMC and external data sources and systems including, but not limited to, clinical process and outcome data, electronic health record data, event reporting data, and patient complaints, etc. (e.g. RL DATIX,Paragon, etc.). Assists in designing, developing, maintaining and updating clinical and other databases based on department and organizational needs.Collaborates on multidisciplinary teams and provides analytical support for various projects related to quality improvement, patient safety, patient experience and outcomes, as appropriate. Interprets, explains and discusses data analysis results.Participates with medical staff leadership and physicians/clinicians to identify and facilitate implementation of best practices and recommend enhancements to processes or stands to support mitigation of risk and improvement of quality.Works collaboratively with healthcare leadership and clinical staff to identify possible or actual risk of harm to patients including the developing and sustaining patient safety programsPrepares and presents patient safety reviews utilizing meaningful written analysis, and data summary reports (charts, graphs, diagrams) to senior leadership and other multidisciplinary meetingsTrack progress and remediation of plan of correction and performance improvement activities related to quality; participating in quality related projects and/or training throughout the organization.Produces reports and presentations on quality and conducts drill down analysis to proactively identify specific variance in practice patterns. Works collaboratively with report requestors, providing guidance to define repot requirements and validate results.Identifies opportunities for operational inefficiencies and collaborates with QA leadership for implementation. Uses Lean principles in work processes.Works collaboratively across departments to understand and meet the organization's clinical quality analytic needs.Meets productivity and timeline targets and uses tools to track activities and timelinesConducts provider on-site audits by researching provider clinical quality issues.Researching provider clinical quality issues and member quality of care grievances, to include outreach to providers across all time zones.Reviewing medical records and pertinent information related to the issue (e.g. claims records).Extrapolates, and summarizing the investigation in written format and preparing case file for clinical quality MD peer-review.Works on issues of diverse scope in which the analysis of data requires evaluation of identifiable factors and in accordance with plan Enterprise and department standards.Documents and logs QOC grievance information on relevant tracking databases and mainframe systems.Maintains hospital quality database via requests for information and questionnaires.Generates appropriate written correspondence to providers, members, and regulatory entities.JOB QUALIFICATIONS
Minimum Education
Bachelor's degree in related health care field required.Preferred Education
