Sentara Health Plans is currently hiring a Patient Services Coordinator! This is a full-time position with day shift hours. Telephonic position focused on outbound calls to Sentara Health Plans members. This is a remote position (candidates must reside in Virginia or North Carolina). Required Qualifications:High School Diploma or EquivalentMinimum 2 years of clinical experience Preferred Qualifications:Telephonic outreach/Call Center experience in a healthcare setting or service-oriented industryPrevious experience in care coordination or patient advocacyProficiency documenting in Microsoft Office Suite applications and electronic health records systemsExperience with motivational interviewing techniques
Primary responsibilities include:Initiating outbound calls to Sentara Health Plans members, screen members identified as an at-risk populationAssisting with gathering information for the health care team regarding barriers or other health informationMotivating members to engage in case management servicesTriaging members to the appropriate team to provide case management servicesDocumenting member interactions, results of screening, and referral to case management in the electronic health recordWorking with members of the professional health care team to coordinate care for members to improve health outcomesMay provide reminders of necessary testing as determined by clinical guidelines to close gaps in careProviding other general administrative functions such as drafting correspondence, scheduling appointments, organizing and maintaining records and files, and providing assistance to membersWorking with members of the professional health care team and with community service agencies to coordinate care for members to improve health outcomeAssists with gathering information for the health care team regarding barriers or other health informationImplements the interventions to promote improved health and compliance with recommended plan of care, as approved by the Health Care Team, including functions associated with the screening for changes in health status, coaching for improved self-management, reminders of necessary testing as determined by clinical guidelines, and support of the professional clinical staff in Health Care ServicesMay serve as a key resource in supporting the clinical authorization processes, coordinating linkage between members, primary care providers, specialty providers and other insurance companiesSupporting the referral process from providers, as well as pre-authorization approvals for medications and other services as ordered/requested by providers
