This is a grant-funded position with an end date of 8/31/25, renewable based upon availability of funding, work performance, and progress toward program goals. 

Purpose

The Pediatric Mental Health Care Coordinator will join a growing multi-disciplinary team in the Department of Psychiatry at Dell Medical School to assist with mental health care coordination for the Texas Child Health Access Through Telemedicine (TCHATT) and possibly other related programs. The TCHATT program provides telehealth services to children for brief mental health assessments and interventions. The purpose of this position is to help children and families navigate through the TCHATT program and assist in connecting to providers in the community for long-term care, if needed. Interest in developing sustainable pathways for referrals and enhancing processes is welcomed.

Responsibilities

Work in an inter-professional model and collaborate with pediatric psychiatrists, psychologists, other licensed professional counselors, and administrative staff to identify the long-term psychological needs of patients. Assemble information concerning patients’ clinical background and referral needs.

Connect students and families to community resources for long term care, providing ongoing support and facilitation to ensure effective transition. Coordinate with TCHATT professionals, schools, and families to minimize obstacles and increase patient and provider satisfaction. Maintain confidentiality of information as appropriate in all aspects of care.

Oversee internal and external communication related to patient’s clinical services, discharge plan and school collaboration among team members, patients and guardians and school staff.  Support team members in disseminating appropriate referral information. Document in the electronic medical record. Collaborate with graduate assistants and/or interns to maintain a community resource database for external referrals, locating student referrals and problem solving complex cases.

Develop and deliver TCHATT trainings for new hires and school staff.

Perform other duties as assigned. 

Required Qualifications

Bachelor’s degree in Social Work, Psychology, or related field. Three (3) years of professional experience working in a health care or educational (preferably k-12) setting. Excellent oral and written communications skills. Able to prioritize, plan, and handle multiple tasks/demands simultaneously. Proficient in Google Suite or other comparable office computer software package.

Relevant education and experience may be substituted as appropriate.

Preferred Qualifications

Master’s degree in social work, psychology, or related field. Knowledge of managed care, medical terminology, patient referral process, claims, and CPT coding. Experience working with pediatric behavioral and mental health patient populations. Knowledge of educational practices supporting mental health and social emotional learning in K-12 settings. Experience developing, refining, and evaluating patient care plans or student education plans.  Experience working in one or more electronic medical records platforms. Bilingual (English/Spanish) language proficiency.

Salary Range

$47,000+ depending on qualifications
