The Pharmacist performs clinical and professionalpharmaceutical work in the compounding and dispensing of drugs and IV supplies.The Pharmacist is responsible for reviewing prescriptions to ensureaccuracy.  The Pharmacist willcollaborate with other healthcare professionals to plan, monitor, review andevaluate patient effectiveness and adverse effects.  The pharmacist provides information andadvice regarding drug interactions and potential side effects as well asanalyze prescribing trends to monitor patient compliance and to preventexcessive usage or harmful interactions.

Essential Duties and Responsibilities:

Prepares, fills and compounds drugs and IV orders byphysicians and other authorized prescribers Maintains narcotic inventory and records usage of allcontrolled substances and pharmaceuticals within the pharmacy Properly stores biological, vaccines and serums May provide specialized pharmaceutical services including,but not limited to, advising physicians on issues concerning drug therapy, theinherent toxicity of drugs and their side effects, as well as assisting in theprescription of appropriate doses Depending upon the assignment, may prepare IV chemotherapysolutions May supervise or coordinate the activities of pharmacytechnicians who are engaged in the preparation, labeling and delivery ofmedications and other pharmaceutical supplies Ensures compliance with federal, state and local laws andregulations pertaining to the dispensing of drugs and controlled drugs and themaintenance of required records When necessary, conducts and evaluates patient medicationhistories, assesses compliance and suggests modifications to medication ordersto achieve desired outcomes Instructs patients in the proper use of prescribed drugs;answers patient questions as needed Individualizes medication regimens using sound principles,accounting for pharmacodynamics and pharmacokinetic variations in drugabsorption, distribution, metabolism and elimination Maintains pharmaceutical knowledge by attending educationalin-services Adheres to all Amergis and worksite rules, policies andprocedures Performs other duties as assigned/necessary

Minimum Requirements:

Bachelor’s degree in Pharmacy from an accredited college ofpharmacy required Current registration and/or license in state of practicerequired One (1) year of experience working as a licensed pharmacistpreferred Complies with all relevant professional standards ofpractice  Participation and completion of Amergis’ Competency programwhen applicable Current CPR if applicable TB questionnaire, PPD or chest x-ray if applicable  Current Health certificate (per contract or stateregulation)  Must meet all federal, state and local requirements  Successful completion of new hire training as applicable tojob site  Understand patient confidentiality and HIPAArequirements   Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required   Computer proficiency required  Must be at least 18 years of age   Duration: 13 weeks or 26 weeks  
Hours: 40
Shifts: Days M-F, rotating weekends
Requirements:
-Pediatric or Oncology experience in Pharmacy
-Hospital experience
