Job Title: Pediatric Physical Therapist
Reports To: Director of Children’s Services
Department: SRVS Kids
Job Purposes (or description): The physical therapist at SRVS Kids is responsible for the evaluation and treatment of gross motor, balance and coordination issues for children birth to 14 years of age. Areas to be addressed include, but are not limited to the following:
Duties and Responsibilities:
- Responsible for the evaluation and treatment of children with delays in gross motor, balance,
coordination, etc.
- Develops and oversees physical therapy goals for the child’s IFSP and/or treatment plan for each
child on their caseload.
- Works with families to create a Home Intervention Plan for carry-over of activities.
- Completes annual re-evaluations and progress reports as needed to prepare for 6 month
reviews, annual IFSPs and IEPs, and help prepare for the transition from Early Intervention
Services.
- Documents each session and therapy report in the electronic health record system and the
TEIDS database (if applicable).
- Provides services and documentation within designated time frame.
- Attends team meetings and collaborates with other team members to meet the needs of each
child and family.
- Complies with all policies and procedures established by SRVS and SRVS Kids.
- Serves as the physical therapist for the Early On program.
Qualifications:
Bachelor’s, Master’s or Doctorate in Physical Therapy, State License and Individual Malpractice
Insurance.
Salary: $60,000
Shift will vary but will get 40 hours per week.
