Job purpose
Our mission is to improve the health and social well-being of the communities we serve through equitable, high quality, patient-centered care that is culturally responsive. We are seeking a Pediatrician (MD or DO) to join our team of outpatient clinicians at Fair Haven Community Health Care. The successful candidate will provide comprehensive primary care services to children from infancy through early adolescence in a fast-paced outpatient setting. This position offers a unique opportunity to make a significant impact on the health and well-being of children in our community.
Reporting to the Site Medical Director, the Pediatrician will ensure timely access to high quality medical care for our patients through evidence-based medical practice and adherence to quality improvement initiatives.
Duties and responsibilities
The Pediatrician will provide primary medical care in accordance with the provider’s medical specialty. Typically, duties include but are not limited to:
+ Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary
+ Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner
+ Confers with other clinicians, patient’s families and other support staff concerning treatment and care of patients when appropriate
+ Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care
+ Maintains accurate and timely medical records within the EPIC electronic health record system; performs appropriate charting and ensures appropriate patient follow-up
+ Participate in quality improvement initiatives to enhance patient care and improve health outcomes
+ Collaborate effectively with other members of the care team, including nurses, medical assistants, social workers, and other specialists
+ Participates in medical & general staff meetings and appropriate clinical teams
+ Participates in the Health Center’s quality improvement program, in other task groups as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care
Qualifications
The successful applicant will have:
+ Graduation from an accredited medical school with a degree of Doctor of Medicine or Doctor of Osteopathy
+ Completion of an approved pediatric residency program and license to practice medicine in the State of Connecticut
+ Knowledge and ability to independently carry out medically based assignments, fill out requisite forms, employing appropriate infection control techniques
+ Strong knowledge base of medical terminology and procedures
+ Ability and willingness to serve on the active medical staff of the hospitals used by FHCHC
+ Qualified in Basic Life Support techniques
+ Bi-lingual in Spanish and English highly desirable
+ Ability to relate with compassion and effectiveness to the patients and other staff of Fair Haven Community Health Care and Strong commitment to providing high-quality, patient-centered care
+ Excellent communication, interpersonal, and organizational skills.
+ Passion for working with underserved populations and promoting health equity.
+ Previous experience in health care setting, ideally in an ambulatory office practice
