Position Summary
Accountable for assisting consumers in the Involuntary Outpatient Commitment program including: ensuring the coordination ofpatient care, Psycho-education, Linkagec to treatment, other services & entitlements. Works collaboratively with the EmergencyServices staff , 2 Central, psychiatry, court system, and the State system of care. Supports the mission, values and philosophy ofHMC.
Primary Position Responsibilities
Accountable for the coordination of patient care for those consumers on the case load though out their involvement in theInvoluntary Outpatient Commitment Program. Reviews records and maintains standards of HMC, JCAHO, DOH, NJ Division of Mental Health & Addiction Services. Provides high quality Case Management and linkage to care. Constructs treatment plans in conjunction with the coordinator and psychiatrist for the court and monitors consumer’scompliance. Constructs Wellness and Recovery Action Plans and Psychiatric Advanced Directives with consumers.

Bachelor’s Degree in Social Work, Psychology, or Equivalent
