Per Diem Case Manager Job Details Job Location CHI- Vanderbilt - Hyde Park, NY Position Type Per Diem Salary Range $21.64 - $24.04 Description JOB SUMMARY Responsible forensuring that all program participants are provided with the services necessary to promote personal and economic independence, to have skills required to obtain, maintain, and retain permanent housing, and are living in well-maintained units. SECTION 1: JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS These duties are essential and specific to the successful implementation of this position. + Adheres to and enforces program regulations, policies, and procedures. Ensures that all clients are receiving quality services that are in compliance with the program. + Assists clients in developing an Independent Living Plan (ILP) by identifying needs and goals, and evaluates customer’s progress on short and long term goals. + Provides information, referrals, counseling, crisis intervention, direct service, and advocacy services + Develops and maintains a resource network for referrals as related to the needs of the individuals being served in supervised programs. Collaborates with DSS/DCFS, property owners, and other service providers to assist Individuals in fulfilling ILP and program requirements. + Assists individuals with direct services such as current and projected budget, current resume as appropriate, assistance with ADL skills, etc. + Responsible for face-to-face meetings with client once a week or more if needed. + Inspects living space regularly as required by the program to assess individual’s ADL needs and program compliance to assist client with maintaining housing. + Limits and documents improper use of agency resources by clients such as unit damages, furniture, or appliances. + Evaluates, documents, and informs Shelter Manager of physical plant needs of the housing unit and/or common areas. + Completes discharge plans with individual. + Submits all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (housing logs, unit inspection sheets, progress notes, ILPs, authorization forms required by DSS/DCFS, special projects, etc.). Maintains charts and files in an orderly fashion. + Other duties may be assigned by management as needed to ensure the successful operation of the program. + Ensures the timely and correct submission of all internal and external reports in the manner required by funding agencies and/or CHI (Census (both CHI and HV), Dashboards, Incident reports (IRC and OTDA), Corrective Action Plan (CAP) housing logs, client contact sheets, unit inspection sheets, progress notes, authorization forms, admission and discharge sheets, census reports, etc Other Duties + Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. Salary range: $21.64 - $45,011.20 to $24.04 - $50,0003.20 Per diem position within Dutchess and Orange county Qualifications ENTRY LEVEL QUALIFICATIONS AND REQUIREMENTS + Education: Bachelor’s Degree in human services or related field, Master’s Degree in Social Work preferred. Extensive experience may be substituted for educational requirement. + Experience: Two years’ experience working in a related field such as homelessness or housing + Communication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish) is a plus. + Computer Skills: Basic competency in MS Windows, MS Office, and on the Internet. + Math Skills: Ability to formulate simple financial budgets and do basic math functions. + Physical Performance: Ability to tour properties, walk distances, climb stairs, etc. + Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls. + Other Skills: Ability to relate to the underserved, homeless and disabled populations. Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details.
