Per Diem Case Manager Registered Nurse (RN), Case ManagementReq #:0000183145 Category:Nursing Status:Per Diem Shift:Day Facility:RWJ Rahway Department:Case Management Location: RWJUH Rahway Hospital, 865 Stone Street, Rahway, NJ 07065 Job Overview: Coordinates quality healthcare services to meet a patient s specific healthcare needs as a result of gathering relevant and comprehensive information by interviewing the patient/family, the primary care physician and the interdisciplinary care team to promote positive clinical and financial outcomes. Performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the patient and those involved in the patient s care. Assures that appropriate services are generated in a timely and cost-effective manner by assisting the patient, whenever possible, to appropriately self-direct care, self-advocate, and make informed healthcare decisions to the highest degree possible. The Case Manager RN will work with a very resilient small group who collaborate with one another and external partners for safe and effective discharge planning. Qualifications: Required: + Baccalaureate degree from an accredited school or university, or equivalent RN degree with commensurate clinical experience + Minimum of 2 3 years experience in a hospital setting + Minimum of one year experience in Case Management/Utilization Management or Home Care + Successful completion of all Orientation Programs Certifications and Licenses Required: + Current Registered Nurse License in the state of New Jersey + Basic Life Support (BLS) from the American Heart Association + CCM certification preferred Scheduling Requirements: + Per Diem + Day shift + 8:30a - 5p + No on call Essential Functions: In accordance with the objective and goals of the Case Management Plan and State and Federal guidelines, the RN Case Manager Discharge Planner will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely manner, and identify the expected length of stay. The RN Case Manager works collaboratively with the interprofessional team, including physicians, social workers, clinical nurses, home care services and others as needed. This role actively participates in specific clinical initiatives focused on reducing the length of stay and continued stay denials, improves efficiency, quality, and resource utilization. The Case Manager RN will work with Medical/Surgical patients.
