CORE JOB SUMMARY The Physician Assistant (PA) delivers patient- and family-centered care in a culturally competent and compassionate manner, utilizing and promoting evidence-based standards of quality, safety, and service while ensuring population-specific patient care. The PA provides and directs hands-on care to patients, practicing within the scope of practice of the Florida Board of Medicine requirements for Physician Assistants, standards of care, and institutional policies and procedures. Within this role, the PA performs all steps of the care process and monitors the quality of care provided. The PA demonstrates expertise and mastery towards patient care, providing care within the philosophy, mission, and objectives of the health system. The PA delivers care that supports the UHealth mission to be a state-of-the-art academic medical center by participating in transforming patient care through innovative research. Diagnostic and Monitoring Functions: Obtains accurate and relevant assessment data and interprets the data as normal vs. abnormal. Determines diagnosis. Modifies patient treatment plans as indicated by patients’ responses and conditions, and collaborates with physicians and other team members. Plans and Implements Therapeutic Interventions: Collaborates with the patient, family, and members of health care team to develop an individualized plan of care including discharge planning. Implements medical interventions safely. Performs appropriate patient tests and safely administers medications within the scope of practice. Administers and maintains accurate records related to medications and treatments as per regulatory bodies, policies, procedures. Evaluates effectiveness of interventions and monitors patient for adverse responses and side effects. Utilizes available technology platforms, and practices and evaluates telehealth services when applicable in accordance with relevant standards. Teaching and Coaching: Patient and Staff Teaching: Assesses a patient's and family's learning needs and readiness to learn. Teaches needed information for self-care and illness prevention. Adjusts information and expectations based on responses from patient, developmental levels, physiological and psychological condition, and cultural variations while addressing and meeting the needs of any language barriers that may be present. Effective Management of Rapidly Changing Situations: Rapidly grasps problem situations and responds quickly and appropriately. Identifies the need for and activates emergency protocols. Serves as a transformational leader in assisting others to adapt to changing work demands and environment. Anticipates possible problems and assists leadership in the development of contingency plans. May participate in activities such as leading an Evidence-based Practice/Performance Improvement project, methodologic approach to develop/submit a research proposal for Institutional Review Board (IRB). Uses best practices for transition of patient care. Meets diverse communication needs of patients with Limited English Proficiency (LEP). Professional Practice Evaluation: Evaluates Advanced practice in relation to professional practice standards and guidelines, relevant statues, rules, and regulations. Participates in shared governance, following the Code of Ethics to elevate the professional, integrating the professional practice model and relationship- based care, delivery model into their practice. Integrates the Magnet Model components into practice. Critical Thinking: Incorporates protocols and standards of care into daily practice. Uses sound judgement, decision making criteria, and available resources for drawing conclusions and taking required action. Applies evidence-based practices, approaches and solves problems with a global perspective using creativity and flexibility. CORE QUALIFICATIONS Education: + Graduate from an Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) accredited physician assistant program by start date. + Master’s degree from an accredited college or university required. Certification and Licensing: + NCCPA certified at time of hire and maintained throughout career at University of Miami. + Full, unrestricted Florida State Physician Assistant License, issued by Physician Assistant Board under Medical Board of Florida by date of hire. + BLS – Basic Life Support certification required + Must have a National Provider Identifier (NPI), or obtain an NPI, prior to employment start date and meet current Medicare requirements for billing. + DEA - Drug Enforcement Administration license (according to department specific requirements) Experience: Preferred minimum of 1-year relevant experience. Knowledge, Skills, and Attitudes: + Strong, effective communication skills with patients, families, and clinical team for innovative patient care and trust + Ability to collaborate with peers in the best interest of the patient to provide consistent quality of care + Ability to adapt and exhibit flexibility to handle new, different, or changing environments + Demonstrates strong time management skills to prioritize, triage, and act proactivity + Ability to uphold professional ethics and maintain patient confidentiality + Knowledgeable on all aspects of patient care including assessing, planning, coordinating, monitoring, and evaluating the patients’ progress through continuum of care Department Specific Functions This per diem position will work closely with the Neurosurgery Faculty, Residents, and nursing staff by assisting the physician in performing surgeries in the main OR and in ambulatory surgery, participating in daily rounds, providing patient care and discharges post-operatively, educating and counseling patients on both an inpatient and outpatient basis, provide support in a clinical setting during patient appointments, and facilitate patient surgical scheduling. The position will also provide research and draft correspondence relating to patient work status, disability, medical prognosis and prospective plans of treatment on both an inpatient and outpatient basis and review patient diagnostics and lab reports for sign off on patient surgical packets and in the clinical setting. Accountabilities (For Non-exempt Employees Include Percent of Effort): 1. Direct Patient Care: + A. Monitors patient care provided by the University of Miami Miller School of Medicine Department of Neurological Surgery Service and takes appropriate actions as required, in multiple settings including surgery, inpatient and outpatient clinics. Communicates and implements plans to all team members, with medical judgment to adjust plans as clinically indicated (ALL changes in plans with be discussed with Attendings) + B. Performs and records complete history and physical examination for new inpatient Neurosurgical admissions. Examines and evaluates patients and assists in development of their treatment plans with the attending physicians. + C. Writes daily progress notes in conjunction w/ Residents and Fellows and dictates discharge summaries. + D. Performs the following patient care functions (but not limited to): suturing, suture removal, wound care/packing, A-line, Central line, NGT/feeding tubes, EVD, and ICP placement and monitoring, lumbar drain placement and lumbar punctures; spine immobilization, draping and patient positioning. + E. Assists the Neurological Surgery Service physician performing surgeries in the main OR suite as well as ambulatory surgery center as needed per Neurosurgery Chief. Assists in outpatient clinics by performing patient histories and physical examinations; organizes diagnostic studies and other pertinent tests, notes patient comments, complaints, and questions, and arranges for patient’s special needs; e.g. physical therapy, surgery, special testing, consultations and follow-up and post op evaluations; and assists in preparation of responses to inquiries from patients, referring physicians and insurance carriers. + F. Participates in daily rounds on Jackson South in-patients with Neurological Surgery Faculty and / or Residents and ARNPs to ensure chart documentation, continuity of patient care and consistency of documentation of data for quality patient care and required research studies, if applicable. Works with patients, family members and other applicable parties on discharge planning and placement. Participates in the daily inter-disciplinary care rounds and collaborates with inpatient nursing services to arrange for needed diagnostic tests and procedures. + G. Arranges and coordinates discharge plans by writing d/c orders, prescriptions, and post-admission teaching for patients and/or families. + H. Prior to discharge, ensures that appropriate follow-up visits are scheduled with the appropriate supervising MDs. + I. Responds to clinical questions from nurses and/or patients in a timely manner. + J. Educates and counsels patients regarding conservative treatment of neurological disorders and the expected outcomes in order to ensure continuity of patient care and consistency of documentation and instructions related to hospital designations and distinctions and research studies, if necessary. + K. Educates and counsels patient requiring surgical intervention, including specific protocols, such as anti-coagulation and physical therapy protocols. + L. Performs lifesaving procedures in accordance with ACLS/ATLS recommendations and departmental protocols/guidelines. 2. Coordination of Patient Care: + A. Works closely with the Chief of Neurosurgery and other Neurosurgery Faculty operating at Jackson South Hospital and provides clinical and operating assistance, and assists in coordinating patient’s admissions and surgeries with the Faculty’s surgical scheduling team. Also assists in planning discharges and providing follow-up care. + B. Arranges, as requested by the Faculty, for patient’s special needs; e.g. therapy, surgery, special testing, and follow-up. + C. Facilitates patient scheduling, for diagnostic and other ancillary testing, as well as arrange patient transfers to the Neurological Surgery Service Unit from other departments, and other hospitals. + D. Assists in preparation of responses to inquiries from patients, referring physicians and insurance carriers. + E. Research and draft correspondence relating to patient work status, disability, medical prognosis and prospective plans of treatment. Categorize and retain data for various research studies. + F. Answers and responds to patient calls within the departmental time frame of twenty four (24) hours or less, instructing and counseling patients on peri-operative and post-operative events and sees patients for both. + G. Reviews all patients’ lab and diagnostic reports. Call patients with results as instructed by the Neurosurgery Faculty. 3. Other duties as assigned and as required by the department based on patient growth and needs. 4. Non-Clinical Duties Team Work, Interpersonal & Communication Skills + 1. Provides “closed loop” communication between all members of the professional and support care staff to ensure that patient care remains the top priority at all times on the service. + 2. Facilitates communication between team members in clinic and on the floors by utilizing any available resources. + 3. Acts as a liaison between staff and patients, communicating updates, treatment plans, follow-up care, testing, etc in a timely and precise manner. + 4. Discusses ideas and concerns with other team members in a “positive feedback” forum that focuses on improving patient care. Leadership Role + 1. Serves as a mentor to PA staff in training, students, and house staff, educating others while continuing to learn and expand one’s own knowledge base. + 2. Behaves as a role model for fellow team members to emulate, and functions as an empathic practitioner. + 3. Demonstrates flexibility and adapts his/her role to whatever task is needed. + 4. Continuously strives to improve the quality of the care given to every patient on the service. Professionalism/Professional Development + 1. Attends all service and multidisciplinary conferences and contributes information and ideas whenever possible. Operations Support/Administrative Responsibilities + 1. Provides continuous feedback for issues, and strives to learn new systems as they become available to improve patients care, documentation, and communication. + 2. Works closely with the billing team to maintain thorough documentation for proper reimbursement and serves as a resource for the hospital in the future for billing as a mid-level provider. + 3. Continues to provide the highest standard of patient care, while incorporating cost-cutting measures and improving efficiency within the department and the hospital. + 4. Provides clinical support for the Neurosurgery Department to improve patient care and satisfaction.
