Description
Fenway Health’s Public Health Sexual Health Clinic is seeking a per diem (7.5-15 hours per week) Sexual Health Provider (NP/PA/MD/DO) to provide direct medical services to public health patients. The clinic is a grant-funded low-threshold sexual health clinic that provides HIV and STI testing and treatment, HIV prevention (PrEP), and counseling for the LGBTQIA+ community. Providers are expected to provide testing and treatment for HIV and STIs in a low-threshold sexual health clinic.
Representative Duties:
Provides direct medical services to Public Health Clinic patients
+ Takes appropriate medical history
+ Conducts physical exam
+ Orders and interprets appropriate tests
+ Formulates differential diagnosis and treatment plans according to accepted standards
+ Prescribes medication according to Fenway policy
+ Refers to medical and/or surgical specialists for consultation as appropriate and follows up on results
+ Discuss HIV/HCV/STI prevention and treatment information with all clients.
+ Build and maintain relationships with Fenway Health medical providers, Public Health staff from all programs, and community-based service providers.
+ Communicate regularly with Public Health and Medical staff in order to share up-to-date information about progress, client barriers to care and issues impacting the community.
+ Communicate with DPH Epidemiologists on positive Syphilis and HIV cases.
Provides psychosocial screening and evaluation to patients
+ Provides preventative health care advice, assesses the psychosocial impact of illness and treats or appropriately refers the patient for evaluation and treatment
+ Assesses the social status of the patient and refers for appropriate intervention as necessary
+ Screens for substance abuse, tobacco use, intimate partner violence, immunization status, nutrition and exercise as necessary and initiates appropriate interventions
+ Provides age-specific care per Fenway Community Health policy
Competently performs a variety of medical procedures when indicated
+ Performs incision and drainage
+ Performs cryotherapy with warts
+ Performs minor surgical procedures
Provides appropriate documentation and credentials
+ Maintains patient records in a timely manner per Fenway policy in the electronic medical record
+ Completes forms and reports for insurance companies and other agencies, as needed
+ Maintains appropriate state licensures for MA
+ Maintains controlled substance licensure in MA and DEA license
Continuing education/program participation
+ Completes minimum CEU requirements
+ Participates in educational conferences with other practitioners
+ Actively participates in educational efforts through HMS and BIDMC
+ Participates in outreach programs
+ Attends standing public health team meetings, contributes ideas, knowledge, and feedback to continually improve the program and clinic
Meets Agency Participatory Expectations
+ Suggests and participates in continuing quality improvement efforts
+ Adheres to all agency and departmental policies and procedures
+ Participates in quality assessment and improvement activities as requested
+ Adheres to the highest principles of patient and client confidentiality
+ Adheres to established safety policies, procedures and precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation
+ Attends all required meetings, in-services and professional trainings
+ Attends all regularly scheduled research meetings, trainings, forums and pertinent FCH staff meetings
+ Maintains professional competence necessary to perform job responsibilities; maintains and provides agency with records of continuing education activities
+ Serves on agency committees and in professional organizations when requested
Performs other related duties as required
Requirements
+ MD, DO, PA, or NP degree
+ At least 2 years of clinical experience as an NP, PA, Do or MD in primary care, Sexual Health, or HIV care
+ Ability to work harmoniously and effectively with colleagues, patients, clients and vendors across the spectrum of diversity, including but not limited to race, ethnicity, color, gender identity, sexual orientation, age, socio-economic status, national origin and immigrant status, religious or spiritual identity, disability (physical, mental, emotional and developmental), veteran status, and/or limited English proficiency.

Preferred Qualifications:
+ Bi-lingual in English/Spanish/Portuguese or other languages
+ Experience operating within a low threshold, walk-in model clinic setting
+ Experience providing sexual health care to underserved populations
+ Knowledgeable about sexually transmitted infections and their treatment
+ Knowledgeable about HIV treatment and HIV prevention
+ Knowledgeable about the care of patients with substance use disorders and those experiencing homelessness.
+ Harm Reduction work experience preferred
+ Experience working in an ethnically culturally and racially diverse staff preferred
Supervisory Responsibility
+ Provide supervision to medical assistants, public health nurses, public health navigators, Research RN’s
+ Provide supervision to Nurse Practitioner students who may rotate through FCHC as part of their training
Working Conditions and Physical Effort:
+ Work involves considerable exposure to unusual elements such as extreme temperatures, unpleasant odors and/or loud noises
+ Moderate physical activity Requires handling average-weight objects up to 15-20 pounds, assisting with patients and standing and/or walking for most the day
+ May work with blood or blood-borne pathogens and will require OSHA training
+ Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment
