Perinatal Coordinator - Meridian & Lake City - 1.0

The Perinatal Coordinator position is part of the OB Program at NCH. Provides program support appropriate for non-licensed provider including: initiating OB patient intake, taking patient histories; providing basic patient education and screening; coordinating transfer of patient and program information between Neighborcare, Kaiser, Swedish, and other institutions; assist in standardizing OB care and systems across sites. Work in partnership and joint accountability with other team members to achieve Neighborcare Mission, Guiding Principles and productivity goals.

In this position you will:

Meet with OB patients for their intake appointment and 2nd and 3rd trimester teachings; provide orientation to prenatal care at Neighborcare; gather complete OB history; refer patient to appropriate resources; and provide basic teaching regarding prenatal care and childbirth. Facilitate, track and document referrals to dental, behavioral health, WIC and other MSS services internally and externally (outside of Neighborcare Health). Process and coordinate outside referrals to OB/GYN, ultrasound, perinatologist, or other specialist, doula services, and/or PH/NFP nurse. Assist OB provider in clinical follow-up: provide basic follow-up regarding prescriptions, lab results and prenatal care options. Consult with OB team when clients “no show”, do not maintain usual perinatal care, and/or are lost to care. Follow-up as instructed. Track patients throughout all parts of the pregnancy including; prenatal, deliveries, postpartum care, transfers, SABS, TOPS, MABS, lost to care and post-delivery visits. Assist in the transfer of prenatal records to Swedish and the collection of labor/delivery/postpartum and newborn records from Swedish back to NCH clinics. Collect basic data using NextGen and SharePoint, as needed for OB tracking. Assist Leadership in promoting OB program at Neighborcare and in the community.

Key Skills, Knowledge & Abilities

Experience using Electronic Health Record systems.

Required Qualifications

2-year degree in health or human services related field or equivalent relevant related experience in a health care setting.

Preferred Qualifications

2-years related work experience with OB/Midwifery care team.

Compensation 

The wage range for this position is minimum $19.97 to 27.25 maximum.  

Compensation will be determined based on years of relevant experience.
