Primary Function:

The Pharmacists in the Logistics Center will assist with Supply Chain, pharmacy automation data base management, automation optimization, and supply chain inventory management. The pharmacists will also help to optimize medication purchasing, inventory accountability, supply, distribution, and control processes. This position is day/evening rotating shifts and works weekends as required.

Principal Duties and Responsibilities:Check medication output distributed from the Pharmacy Consolidated Service Center (PCSC) automation including PillPick®, Boxpicker®, AutoPack® and other automated devices as well as technician prepared kits and/or packaged medicationsCheck bulk medication for stocking in PCSC automationSupport the creation, management, maintenance, and troubleshooting of supply chain databasesEnsure all hazardous medications are being handled correctlyProvide coverage for callouts, FMLA, PTO, and other absencesReview and interpret prescriber medication orders, detect and evaluate adverse drug effects, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/ precautions, the patient’s current status of health and review drug profiles and patient’s medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentialsResolve any medication related issues in consultation with the prescriber and nurse and clarify questioned orders prior to dispensing the first doseVerify medication orders in the PharmNet computer database and assure these orders are carried out accurately and efficientlyEvaluate specific medication therapies consistent with departmental policies (e.g., aminoglycoside and vancomycin dosing by pharmacy)Assure that appropriate computer reports and batch fills are run according to scheduleConvert and manage medications per P&T approved pharmacist policies for patientsCollect and document system problems identified by staff and refers to Supply Chain Leadership for direction and resolutionAssure handling of medications throughout the hospital follows state and federal laws and regulations and hospital policiesParticipate in identifying and submitting Report 2 Learn (R2L) documentation and providing follow up assessment on all encountered adverse drug events, medication errors and other related safety issues or learning opportunitiesAdvise, assist, and supervise work of pharmacy technicians and assume final responsibility for accuracy of their workAssist with training programs of pharmacy interns, residents, students, and technicians; provide information in all pertinent areas of pharmacy practices, policies, and proceduresEducation and Experience Requirements:BS, MS or PharmD in Pharmacy requiredASHP accredited PGY-1Pharmacy Practice Residency preferredPrior hospital pharmacy experience preferredCurrent pharmacy licensure or eligibility for licensure in the state of DelawareChristianaCare Offers:Full Medical, Dental, Vision and other insurance benefits403 (b) with an employer matchGenerous Paid Time OffIncredible Work/Life benefits including annual membership to care.com, access to backup care services for dependents through Care@Work, retirement planning services, financial coaching, fitness and wellness reimbursement, and phenomenal discounts through several vendors for hotels, rental cars, theme parks, shows, sporting events, movie tickets and much more!
