Pharmacist

Position Summary: The pharmacist ensures the safe and effective use of medications by providing medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration, and drug therapy monitoring services. Monitors inpatient and outpatient drug therapies when appropriate, and works with relating medical staff to find alternatives when needed, to ensure the best care possible is being given.

Responsibilities: 

Receives and interprets medication orders and transcribes to computerized patient medication profiles. Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products. Issues controlled substances to patient care areas and maintains records. Completes pharmacokinetics consults ordered during their shift determining medication dosing. Provides information and education patient/family regarding medications. Provides clinical consultation and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives to medical staff.

Qualifications: 

Required:

Graduate of a School of Pharmacy (Pharm D or BS Pharm) accredited by the Accreditation Council for Pharmacy Education (ACPE) Licensed with the State of Kansas Board of Pharmacy. Knowledge of principles, methods and techniques involved in performing general pharmacy services and adapting or modifying standards of pharmacy practices for care of specific medical cases.

Infection Control: Initial and Ongoing training in dealing with infection control. Trainings could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.

Patient Interaction: Periodic 

HIPAA: This position will have access to the following Protected Health Information in order to carry out the duties related to their position at Hays Medical Center based on the following criteria:

Primary – required (routine) to do the job;

Secondary – required for the job, but mostly be exception; and

None – no approved access

Primary:

Patient Demographic Information (information used to identify a person): Name, Date of Birth, Address, Race, Marital Status, Religion.

Financial Information/Insurance (information related to insurance, billing and payment): Billing Information, Payer Name, Payer ID, Account Balances, Plan Elements Covered, Payment Information, Payment Rates.

Coding Information (clinical information that is in (alpha) numeric format): ICD-9 Codes, Rev Codes, CPT Codes.

Clinical Information (information that describes a patient’s health status): Diagnosis, Reports/Medical Notes, Test Results, Problem List, Procedures, History and Physical.
