Job Description
As a Pharmacist, you will evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. You will make therapy change recommendations to physicians and facility staff as appropriate. You will maintain a patient/resident database including current and past medical history, clinical and laboratory data, and concurrent medication. You will ensure that appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team. Collaboration with the healthcare team to provide clinical and technical support as needed is essential.
ResponsibilitiesEvaluate physician medication orders for drug appropriateness, dosage, potential interactions, and administration route.Make therapy change recommendations to physicians and facility staff.Maintain patient/resident database including medical history, clinical and laboratory data, and concurrent medication.Ensure appropriate laboratory tests for high-risk conditions are ordered and evaluated.Collaborate with healthcare team to provide clinical and technical support.Document all communications with healthcare team members and patients/residents in the medical record.Implement policies and procedures for safe drug procurement, storage, distribution, use, and disposal.Maintain all required documentation in accordance with federal and state regulatory agencies and policy.Inform Pharmacy Manager of medication-related errors.Assist facility/agency staff with safe and effective drug procurement, maintenance, administration, and disposal procedures.Perform other tasks as assigned.Essential SkillsBachelor's Degree in PharmacyPharm D preferred1+ year experience in long-term careValid/Active Pharmacist license in GeorgiaKnowledge of pharmacy laws, federal and state regulations, pharmacy benefits management, and closed-door pharmacy operationsAdditional Skills & QualificationsGoal and detail orientedTeam playerGood oral communication skillsWork Environment
This role operates in a closed-door pharmacy environment, which is calm and quiet. Shifts will be from 11:00am - 8:00pm and 2:30pm - 11:00pm.
