Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.* 2. Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles.* 3. Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques.* 4. Issues controlled substances to patient care areas and maintains records as required by law.* 5. Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.* 6. Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.* 7. Assists with pharmacokinetics consult service and drug dosing per organizational protocols.* 8. Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed. 9. Reads, extracts, and interprets information in patient charts accurately.* 10. Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.* 11. Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selection.* 12. Provides clinical consultation and clarification to practitioners. Recommends evidence-based medication therapy regimens and monitoring plans. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed. 13. Provides accurate, adequate, and timely drug information to the professional staff.* 14. Provides drug information to patients and their families. 15. Documents all clinical activities and interventions accurately and completely.* 16. Participates in the quality improvement and medication-use review activities of the department. Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned.* 17. Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs. 18. Contributes to the quality and effective operation of the department.* 19. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.* 20. Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window. 21. Keeps pharmacy areas and equipment clean, neat, and well organized. 22. Performs essential duties of the pharmacy director in his/her absence. 23. Maintains current pharmacist licensure. 24. Attends orientation, education, and training programs. 25. Reviews literature and other materials pertinent to the practice of pharmacy. 26. Completes all competence/skills assessment requirements.* 27. Attends meetings and participates in committees as required 28. Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window. 29. Adheres to health system and departmental policies and procedures. Complies with all requirements related to risk management, safety, medication-use safety, security, fire safety, and infection control. Complies with all applicable federal, state, and local laws, rules, and regulations. 30. Maintains strict confidentiality of patient, visitor, and employee information. Complies with HIPAA Privacy Rule standards. 31. Performs work within specified time frames. Adapts positively to frequent interruptions and changes in workload and/or work schedule. 32. Provides courteous, cooperative service to patients, visitors, and staff. Demonstrates good oral and written communication. 33. Works cooperatively with all staff. Voices concerns and suggestions to appropriate persons in a positive manner. 34. Establishes and maintains long-term customer relationships, building trust and respect by consistently meeting and exceeding expectations 35. Demonstrates proficiency in required operating systems: (i.e. Meditech, MedCom, Automated Dispensing Machines, Clinical Pharmacology, Microsoft Office, etc.) 36. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner. 37. Understands and meets customers’ needs and expectations. The patient and family members always come first. 38. Articulates knowledge and understanding of department and facility policies, procedures and systems 39. Answer telephones, responds to emails, research information, utilize drug resources 40. Is punctual and dependable; reports to work as scheduled. Fulfills on-call obligations per prearranged schedule. Absenteeism and tardiness are within policy guidelines. 41. Maintains a neat, professional, well-groomed appearance. Wears identification badge at all times. 42. Demonstrates sound professional judgment consistent with clinical/academic background. 43. Fosters a team environment by providing orientation and training to new team members. 44. Performance demonstrates efforts to improve patient satisfaction, lower costs, improve quality, and promote safety. 45. Demonstrates the ability to address problems in a group setting using tools and techniques for identification and resolution of problems. 46. Demonstrates the values and behaviors of the organization. 47. Participates in maintaining department inventory levels 48. Orders medication based on PAR levels 49. Orders medication based on clinical need in conjunction with formulary initiatives 2 50. Works with DOP or designee as needed in inventory management initiatives 51. Participates in maintaining department formulary initiatives 52. Communicates with appropriate medical, nursing and allied health professional staff members with regard to non-formulary decisions 53. Works with DOP or designee as needed in formulary clinical review process 54. Perform other duties as deemed necessary by the Director of Pharmacy/Clinical Manager in accordance with established facility hospital policies and accrediting agencies. QUALIFICATIONS EXPERIENCE EDUCATION Preferred: Health-system experience Residency training or equivalent experience Preferred Bachelor of Science in Pharmacy from an accredited school of pharmacy Current license to practice pharmacy in the Commonwealth of Virginia required.
