Job Title: Pharmacist
Job Description:
We are seeking a dedicated and detail-oriented Pharmacist to join our team. The Pharmacist will be responsible for evaluating physician medication orders, making therapy change recommendations, maintaining patient databases, ensuring appropriate laboratory tests are ordered, and collaborating with the healthcare team. This role involves implementing policies for the safe procurement, storage, distribution, and use of drugs, documenting communications, and assisting staff with drug-related procedures.
Responsibilities:
+ Evaluate medication orders for appropriateness.
+ Recommend therapy changes to physicians and staff.
+ Maintain patient databases with medical history and lab data.
+ Ensure necessary lab tests are ordered and discussed with the team.
+ Collaborate with the healthcare team for support.
+ Document communications in medical records.
+ Implement drug safety policies and maintain documentation.
+ Inform the Pharmacy Manager of medication errors.
+ Assist staff with drug procurement, maintenance, administration, and disposal.
+ Perform other assigned tasks.
Essential Skills:
+ Bachelor’s Degree in Pharmacy; Pharm D preferred
+ Tennessee Pharmacist License.
+ One year of long-term care experience preferred.
Work Environment:
+ 10:00am-7:00pm / Monday to Friday , with possible weekend shifts!
+ This is a non-retail, closed-door pharmacy focused on long-term care.
