Job Title: Pharmacist

Job Description

We are seeking a dedicated and detail-oriented Pharmacist to join our team. The Pharmacist will be responsible for evaluating physician medication orders, making therapy change recommendations, maintaining patient databases, ensuring appropriate laboratory tests are ordered, and collaborating with the healthcare team. This role involves implementing policies for the safe procurement, storage, distribution, and use of drugs, documenting communications, and assisting staff with drug-related procedures.

ResponsibilitiesEvaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.Make therapy change recommendations to the physician and facility staff as appropriate.Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.Collaborate with the healthcare team to provide clinical and technical support as needed.Document all communication with physicians, nurses, other healthcare team members, and patients/residents and their family members in the medical record.Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal, maintaining all required documentation in accordance with federal and state regulatory agencies and policy.Inform the Pharmacy Manager of medication-related errors.Assist facility/agency staff with procedures related to the safe and effective methods of procuring, maintaining, administering, and disposing of drugs.Perform other tasks as assigned.Essential SkillsBachelor's Degree in Pharmacy; Pharm D preferred.Tennessee Pharmacist License.One year of experience in long-term care preferred.Additional Skills & QualificationsDetail-oriented.Goal-oriented.Team player.Strong oral communication skills.Work Environment

This is a NON-RETAIL, closed-door pharmacy environment focused on fulfilling the pharmaceutical needs of long-term care. The shift for this position is 10am-7pm Monday to Friday, with possible weekend shifts.
