Clinical Pharmacist

TOC RPH will have rotating shift that supports the

following:

-Inpatient service lines:

cardiology, heart failure, neurology, medicine, geriatrics, psychiatry,

hematology, oncology, others

-Post-discharge transitions of care

clinic

-Admission and discharge medication

reconciliation

-Patient/caregiver education

-Ensure medication access for

patients

-Precepting of residents/students

-Projects and assignments as

assigned

-Prior Authorization Consultation

Service 

Requirements:

Licensed Pharmacist in the State of CA1 year (minimum) of inpatient pharmacist experience with medication reconciliation Current BLS

Schedule: 5x8 Monday - Friday primarily. Will be required to work 1 or 2 weekend days a month. Candidate must have availability for the 1st and 2nd shift. first shift starts at 8am. second shifts starts at 1pm.

Pay and Benefits

The pay range for this position is $60.00 - $70.00
