Job Description
We are seeking a dedicated and experienced pharmacist to join our team. The ideal candidate will evaluate physician medication orders for appropriateness, maintain patient and resident databases, ensure proper laboratory tests, document communications, implement safety procedures, and provide support to the healthcare team. This position requires a proactive individual who can work independently in a long-term care, close-door pharmacy environment.
ResponsibilitiesEvaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.Make recommendations for therapy changes to the physician and facility staff as appropriate.Maintain patient and resident databases including medical history, clinical and laboratory data, and concurrent medications.Ensure appropriate laboratory tests for high-risk conditions are ordered, evaluated, and discussed with healthcare team members.Document all communications with physicians, nurses, other healthcare team members, and patients or residents and their family members in the medical record.Implement policies and procedures for the safe procurement, storage, distribution, and disposal of drugs and biologicals.Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and company policy.Inform the Pharmacy Manager of medication-related errors.Assist facility or agency staff with procedures relating to safe and effective drug procurement, maintenance, administration, and disposal.Conduct job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct and Ethics, policies and procedures, the Corporate Compliance Agreement, applicable laws, and professional standards.Perform other tasks as assigned.Essential SkillsPharmacy LicenseBachelor’s Degree in Pharmacy (Pharm D preferred)One year of experience in pharmacy (long-term care preferred)Knowledge of pharmacy laws and federal and state regulations concerning long-term careAbility to work cooperatively as a member of a teamAdditional Skills & QualificationsBusiness knowledge and understanding of operational aspects of the businessKnowledge of pharmaceutical care practices for long-term care residentsCurrent MD State LicenseWork Environment
This role operates in a close-door pharmacy focused on long-term care. The facility is a 24-hour operation, with shifts from 7 PM to 7 AM, working 7 days on and 7 days off. The pharmacist will be the only pharmacist on duty overnight and must be a self-starter capable of working independently.
Pay and Benefits
The pay range for this position is $59.00 - $64.00
