SUMMARY (BASIC PURPOSE OF THE JOB)Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols. Contributes to the quality and effective operation of the department. Demonstrates proficiency in information systems, including clinical information system and automated dispensing system. Effectively manages site-specific Outpatient Prescription Program. Ensures understanding of Federal and New Jersey laws governing controlled substance dispensing. Prepares sterile products according to policy. Prepare NICU and pediatric doses according to hospital policy.MINIMUM REQUIREMENTS

Education: Bachelor’s degree in pharmacy. PharmD graduate of an ACPE Accredited School of Pharmacy preferred.

Experience: One year of hospital related pharmacy experience preferred.

Other Credentials: AHA BLS - Healthcare Provider,Pharmacist - Registered

2 hours of Stroke related Continuing Education contact hours annually

Knowledge and Skills: Excellent verbal and written communication skills; Strong attention to detail; Knowledge of and ability to use computer or typewriter keyboard; Knowledge and skills necessary to provide care appropriate to patients of all age groups served by the Pharmacy.

ESSENTIAL FUNCTIONSProcesses medication orders via CPOE or directly into the clinical computer system that includes critically evaluating drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose documented by intervention reports. Detects and reports suspected adverse drug events accurately and in a timely manner by documentation of ADE reports.Uses OmniLink system to prioritize orders and to document unresolved medication interventions effectively.Provides clinical and direct patient care services for assigned population(s) or floor(s)/unit(s). Responds to consultation for pharmacy interventions and make appropriate evidence-based, patient-centered medication therapy recommendations. Works as an active member of multidisciplinary team and collaborate with healthcare providers within patient care area. Attends multidisciplinary work/care rounds with designated service as appropriate. Recommends appropriate, cost effective therapeutic alternatives to medical staff as needed.Manages the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection by processing non-formulary drug requests and documentation in the clinical intervention log.Answers the telephone in a courteous and timely manner and identifies self by name when taking calls. Responds or redirects requests as appropriate, as evidenced by personal monitoring and number of complaints received.Supervises and directs pharmacy technicians and pharmacy support personnel as evidenced by co-signatures. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.Participates in department quality improvement programs as evidenced by documentation of meeting attendance and reports produced.Completes and documents inspections of all assigned medication storage areas according to published schedule. Identifies and replaces outdated and unusable drugs as documented on unit inspection forms.Monitors drug and ancillary supplies and ensures that special ordered items are brought to the buyer's attention timely. Dispenses accurately medications by matching the physician's order to the label, and verifying the "five rights" patient, drug, dose, route and frequency prior to dispensing. Ensures drug dispensed is 'in date'.Keeps pharmacy work areas and equipment clean, neat and well organized as evidenced by inspection. Disposes all hazardous waste in appropriate hazardous substance containers.Generates reports in accordance with established procedures. Generates fill lists, label runs, production lists, and profiles. Implements or describes backup plan in event of power or communications failure in accordance with established procedure. Troubleshoots problems as needed.Performs all controlled substance transactions accurately using the Omnicell Controlled Substance Manager (CSM) platform. Reviews the daily inventory log, checks inventory, investigates discrepancies. Reports all unresolved discrepancies to management. Understands the CII safe procedure for removing controlled substances from the inventory for DEA return or destruction. Properly stores expired controlled substances to prevent loss. Refills and checks the inventory of anesthesia narcotic boxes accurately.Completes successfully initial and ongoing aseptic technique, USP <797>, and USP <800> assessment program. Compounds sterile products as necessary. Demonstrates understanding of hospital policy and procedures for hazardous drug and antineoplastic preparation.Ensures that labels contain appropriate drug information, documentation and use by dating. Follows all quality assurance procedures during the compounding process and effectively use TPN software and compounder (if necessary). Employs the double check system to ensure correct calculations, dose, volume, drugs and label. Employs on line resources, calculators to determine appropriate dosing, and labs.Follows policy for dilution, compounding of all pediatric, and NICU doses. Employs the pharmacist double check system prior to mixing all dilutions. Performs final quality assurance check initialing each dose on the log prior to dispensing.Processes employee prescription properly into the outpatient computer program in compliance with the procedures and the applicable NJ and federal laws and regulations as needed. Evaluates critically the eligible employee and/or dependents profiles, generates labels and accurately dispenses the medications. Checks technician dispensing where appropriate. Discusses and provides drug information to employees upon requests or where appropriate. Provides great customer services including contacting the employees. Understands federal and New Jersey State laws governing outpatient prescriptions. Assures prescriptions and patient profiles contain complete information. Offers patient counseling. Ensures transmission, pick-up and delivery of prescriptions between campuses.PHYSICAL DEMANDS AND WORK ENVIRONMENT
