Job Description
We are seeking a dedicated and experienced Pharmacist to join our team. This is an onsite position with working hours from Monday to Friday, 10:00 AM - 7:00 PM CST, and occasional Saturdays (8-10 times a year) from 8:00 AM - 5:00 PM. Pharmacists will receive a day off during the week when working on Saturdays. This role involves extensive phone communication, counseling patients about their medications, and verifying prescriptions. This is a contract-to-hire position.
Responsibilities
+ Oversee pharmacy technicians.
+ Conduct comprehensive clinical reviews of pharmacotherapy for each patient, including drug utilization, counseling, compliance, drug interaction screening, side effect profiling, and drug administration support.
+ Document pharmacotherapy review results in patient charts to inform the nursing team.
+ Counsel patients on new drug therapies in accordance with clinical management models.
+ Complete REMS training and follow program SOPs accordingly.
+ Coordinate patient care with the referring clinician, addressing social, psychological, and physical needs, and document these services in progress notes.
+ Serve as a liaison between patients, caregivers, home health agency nurses, hospice nurses, and the prescribing physician/clinician.
+ Provide subcutaneous teaching support for patients and caregivers, and coordinate nursing services if necessary.
+ Provide on-call coverage as directed by the Pharmacist-in-Charge.
+ Process prescriptions accurately and timely.
+ Verify the discontinuation of service for each patient and deactivate the patient's chart, prescriptions, and software profile appropriately.
+ Implement clinical management models for products and disease states.
+ Act as the primary intake for all new team referrals initiated by telephone.
+ Commit to providing exceptional customer service.
+ Build and foster relationships with patients and clinicians.
Additional Skills & Qualifications
+ Experience in retail pharmacy is advantageous.
+ Experience in independent pharmacy is beneficial.
+ Ability to handle and rectify errors and missed metrics effectively.
Work Environment
+ This is an onsite position located at 13501 Park Vista Blvd Suite 150, Ft Worth, TX.
+ The working hours are Monday to Friday, 10:00 AM - 7:00 PM CST, with occasional Saturdays (8-10 times a year) from 8:00 AM - 5:00 PM. Pharmacists will be on the phone for most of the shift, counseling patients and verifying prescriptions.
+ The role requires strict adherence to company policies, especially those related to government-funded payers, Medicaid, and Medicare.
+ The environment is professional, with a focus on providing exceptional customer service and maintaining compliance with all relevant laws and regulations.
Pay Rate: $60/hr

The pay range for this position is $60.00 - $60.00
