Pharmacist (100% Remote) Job Description The clinical pharmacist will participate in a wide range of provider, patient, and employer initiatives and programs. This role supports various lines of business including Medicare, Commercial, Exchange (Marketplace), and Medicaid. The primary focus will be on ensuring quality, appropriateness, safety, and cost-effectiveness of services for these member populations. Additionally, the MTM Pharmacist will conduct telephonic consultations to provide education, coaching, and identify opportunities to improve patient care. Responsibilities + Participate in provider, patient, and employer initiatives and programs. + Support Medicare, Commercial, Exchange (Marketplace), and Medicaid lines of business. + Manage quality, appropriateness, safety, and cost for member populations. + Review targeted patients' prescription and medical claims history. + Conduct telephonic consultations with members to provide education and coaching. + Identify potential opportunities to improve patient care. + Interact with prescribers to facilitate appropriate changes to medication regimens. + Conduct prior authorization reviews for self-administered weight loss and diabetes drugs. Essential Skills + Pharmacy Degree and NY State Licensure. + Internship experience in an organized health care setting or practice environment with direct physician interaction. + Strong customer service orientation. + Fundamental knowledge of drug information process/references. + Ability to conduct research. + Strong clinical pharmacy experience. + Proficiency in Microsoft Word and Excel. + Excellent verbal and written communication skills. + Ability to conduct a thorough drug regimen review. + Maintains confidentiality and uses only the minimum amount of protected health information (PHI) necessary. Additional Skills & Qualifications + Basic knowledge of CMS minimum standards for MTM services is desired. + Open to years of experience. + Can be PharmD or Bachelor of Science. + Managed care experience is a plus but not required. + Retail pharmacy experience is welcome. + Quick learner who can adapt to new systems quickly. Work Environment Work hours are from 8:30 AM to 5:00 PM, Monday to Friday, with no weekends required. This is a 100% remote position, though occasional travel may be necessary. The role involves significant time spent at a computer and using the telephone. The environment is fast-paced, requiring the ability to manage multiple projects simultaneously while maintaining attention to detail and accuracy. There is frequent interaction with external customers and high-profile contact with outside vendors, providers, and customers at the highest levels. Ability to travel frequently across New York State and neighboring areas is required, with occasional travel out of the area. Pay and Benefits The pay range for this position is $45.00 - $50.00 Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
