Job Description
We are seeking a dedicated and detail-oriented pharmacist to join our team. The ideal candidate will evaluate physician medication orders for appropriateness, make therapy recommendations, and maintain patient databases. The pharmacist will collaborate with the healthcare team to provide both clinical and technical support, ensuring the safe and effective use of medications.
ResponsibilitiesEvaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.Make recommendations for therapy changes to the physician and facility staff as appropriate.Maintain patient/resident database, including current and past medical history, clinical and laboratory data, and concurrent medications.Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.Collaborate with the healthcare team to provide clinical and technical support as needed.Document all communication with physicians, nurses, other healthcare team members, patients, and their family members in the medical record.Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.Inform the Pharmacy Manager of medication-related errors.Assist facility/agency staff with procedures related to the safe and effective procurement, maintenance, administration, and disposal of drugs.Essential SkillsPharmD or Bachelor's in Pharmacy (PharmD preferred).Minimum of 1 year experience in long-term care.Pennsylvania license required.Experience with medication refills, inventory management, and data entry.Additional Skills & QualificationsWill consider candidates with a Bachelor's in Pharmacy and relevant experience.Experience working in a long-term care facility is advantageous.Work Environment
The position is in a closed-door pharmacy that is fast-paced and high-volume. The shift is overnight from 7:00 PM to 3:30 AM, and flexibility for weekends is required. This role offers the opportunity to work in a 503B closed-door pharmacy that supports long-term care facilities.
Pay and Benefits
The pay range for this position is $60.00 - $60.00/hr.
