**General Summary/Responsibilities:** Department: MED THERAPY MGMT SERVICES MVHS Shift: 0700 - 1530 Status: Full Time / 80 hours per pay period Facility: Miami Valley South The Ambulatory Clinical Pharmacist provides pharmaceutical care services by direct interaction with patients and providers. The pharmacist will spend a majority of his/her time in patient related activities. These activities require a great degree of skill in patient monitoring, drug therapy review and assessing patient outcomes. The pharmacist performs a full range of professional practices of pharmacy based upon his/her Scope of Practice and serves as a resource to other ambulatory practice clinics and hospital staff. **Qualifications** **Education** Minimum Level of Education Required: Bachelor's degree **Additional requirements:** Type of degree: Pharmacy Area of study or major: Pharmacy Preferred educational qualifications: PharmD and Pharmacy Residency preferred Position specific testing requirement: N/A **Licensure/Certification/Registration** · Licensed Pharmacist · License is in good standing with the Ohio Board of Pharmacy · Certified Anticoagulation Care Provider (CACP) or (equivalent certification) or Board Certified Ambulatory Care Pharmacist (BCACPS), or Board Certified Pharmacotherapy Specialists (BCPS) **Experience** Minimum Level of Experience Required: 1 - 3 years of job-related experience Prior job title or occupational experience: Hospital or Ambulatory Care Pharmacist Prior specific functional responsibilities: N/A Preferred experience: residency trained and/or board certified Other experience requirements: N/A **Knowledge/Skills** Proficiency with computer systems and PC applications is required. The ability to assimilate and effectively utilize a broad range of pharmaceutical and medical data, procedures, and terminology is required. The ability to organize and supervise pharmacy operations and associated personnel is a necessity with minimal direction. The incumbent must demonstrate evidence of skill in pharmacy operations. The ability to act as a liaison between professional and nonprofessional personnel is required.
