Pharmacist - Anticoagulation Clinic - PRN
**Employment Type:** Part time **Shift:** Day Shift **Description:** **Position Purpose:** The Anticoagulation Clinical Staff are responsible for the Anticoagulation Clinic Service. The pharmacist assists prescribers in caring for patients on anticoagulation therapy to achieve desired outcomes. **What you will Do:** + Provides oversight and guidance for the Anticoagulation Clinic Service. + Performs drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients. + Provides appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocol and Chest Guidelines + Educates patients and families regarding anticoagulation therapy. + Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes in Coagclinic® and Webcare. + Communicates effectively with prescribers referring patients to the Anticoagulation Clinic. + Maintains competency in anticoagulation care. Serves as an active patient advocate regarding anticoagulation therapy Uses a team approach to working with staff and others **Hours/Schedule:** PRN; as needed. **Minimum Qualifications:** + At a minimum, a BS in Pharmacy from an accredited college of pharmacy.
