Overview Full-time / 1.0 FTE /80 hours biweekly Salaried position Javon Bea Hospital - Riverside Located in Rockford, IL A pharmacist who uses knowledge of current developments in the field of pharmacy to develop and coordinate clinical pharmacy and medication safety programs and initiatives to ensure safe and appropriate medication use processes for patients. Serves as a liaison between pharmacy, nursing, medical staff, and other healthcare professionals in order to ensure safe and standardized medication use processes. Provides professional services that develop staff competencies, such as, coordinating continuing education programs, leading staff activities related to optimization of medication use outcomes and patients safety to ensure that the medication use process meets or exceeds the reasonable expectations of patients and their caregivers. As a member of the pharmacy leadership team assist in the recruitment, supervision and evaluation of pharmacists and ancillary pharmacy staff. Performs pharmacist duties as required. Responsibilities CLINICAL SERVICE COORDINATION: Coordinates floor-based clinical pharmacy services. Directs clinical program development and implementation as needed. Provides education to pharmacists, physicians, nurses, and students pertaining to the pharmacy clinical activities. Provides director with pharmacist assessments pertaining to pharmacy clinical programs. QUALITY IMPROVEMENT FUNCTIONS: Works with director to maintain selected quality indicators. Provides monitoring of drug usage patterns and effectiveness to maintain formulary effectiveness. Provides pharmacy director and assistant director with selected data collection and analysis. Participates in selected case conferences. Promotes effectiveness of formulary while maximizing cost savings opportunities for drug therapies. Maintains appropriate formulary representation of pertinent medication classes use in the facility. Develops pharmacist driving I.V. to P.O. switch programs. Develops automatic substitution programs for appropriate classes of medications. Participates on various Health System committees and makes contributions to physician and nursing newsletters. Attends grand rounds. Participates in nursing unit educational activities as needed. Actively supports clinical pharmacist involvement in nursing unit and other pertinent events. Provides input to pharmacy director to aid with competency assessments. Maintains an acceptable working knowledge of staff pharmacist functions in order to assist with staffing as needed. Education and Experience Graduate of an accredited Pharmacy program. PharmD preferred. Minimum 3 years' experience as a clinical pharmacist. Certification and Licensure Registered pharmacist in the State of Illinois and/or Wisconsin Prefer association with American Society of Hospital Pharmacists and Illinois Counsel of Hospital Pharmacists.
