Position Summary:
Provide outpatient clinical pharmacotherapy, clinical pharmacology, or pharmaceutical services to patients receiving care within their area. Serve as a resource to primary providers. Develop and participate in programs and activities to ensure optimal use of medication therapies. Perform Staff Pharmacist functions as assigned. Ensure adherence to UNMH and departmental policies and procedures and compliance with State and Federal laws and regulations. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Detailed responsibilities:
* PATIENT CARE - Ensure the delivery of optimal written and verbal pharmaceutical care to patients with in their area of specialty
* PATIENT CARE - Participate in the development of clinical protocols or order sets to ensure the appropriate and safe delivery of medications to patients; develop and provide information resources for patients to ensure the optimal adherence to medications prescribed
* QUALITY IMPROVEMENT - Continually evaluate current activities and propose new areas of involvement or changes in current practices
* QUALITY IMPROVEMENT - Develop and participate in programs to improve the quality of patient care and ensure the safe delivery of medications
* QUALITY IMPROVEMENT - Ensure departmental and hospital compliance with national quality standards appropriate to their area of practice (e.g., Joint Commission, CMS, UHC) through documentation of Quality Assurance
* QUALITY IMPROVEMENT - Report and evaluate medication errors and adverse drug events and recommend systems for improving the safety of pharmaceutical care
* TRAINING & EDUCATION - Develop and provide educational programs for Pharmacists, other Healthcare Professionals or Patients in the Community
* TRAINING & EDUCATION - Develop training and competencies and train UNMH Pharmacists and Technicians; other UNMH Healthcare professionals; Pharmacy Residents; and patients
* FINANCIAL - Ensure the most cost-effective delivery of medication therapy and use of medications to UNMH patients; develop and implement programs to promote cost-effective alternatives and track their effectiveness; evaluate opportunities to decrease pharmacy and other healthcare professionals time to provide optimal pharmaceutical care
* TRAINING & EDUCATION - Serve as an advisor or participate on committees that are involved with the education of Students, Pharmacists, other Healthcare Professionals or Patients in the Community
* FINANCIAL - Evaluate opportunities and implement programs to decrease waste or improve reimbursement involving medication therapies
* RESEARCH - Actively support the Research Mission of UNMH and the Health Sciences Center
* RESEARCH - Present research findings in publications of a peer-reviewed professional journal or a professional meeting
* COORDINATION - Work with other departments and healthcare professionals to develop programs to optimize the delivery of pharmaceutical care at UNMH
* COORDINATION - Actively work with other departments and healthcare professionals to optimize the delivery of pharmaceutical care; attends and participate in Department, Hospital, Medical Staff or Health Sciences Center committees as assigned
* PROJECT MANAGEMENT - Design, implement, and report on projects that evaluate or improve the delivery of pharmaceutical care to patients including: Medication Use Evaluations; Formulary reviews; Therapeutic Class Reviews; Medication Safety Projects; Quality Improvement Projects; Performance Improvement Projects; and Cost-Reduction Projects
* QUALITY IMPROVEMENT - Develop and participate in a planned periodic chart review with supervising physician(s) that meets the requirements of the New Mexico Board of Pharmacy
* PATIENT CARE - Evaluate and revise intervention care/treatment plan with patient, family and supervising provider as necessary to achieve improved health
* DOCUMENTATION - Provide accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential records
* PATIENT CARE - Perform non-invasive diagnostic and therapeutic procedures
* PATIENT CARE - Develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statute, UNM Hospitals and Pharmacy Department policy and procedure
* PATIENT CARE - Under Hospitals Bi-Laws and department protocol provide direct patient care by writing and renewing prescriptions
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

* Licensed Pharmacist Clinician in NM w/in 180 days
* CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
