Pharmacist - Emergency Department
Overview We have a 0.7 FTE Clinical Emergency Medicine Pharmacist position available at M Health Fairview Ridges Hospital! The Clinical EM Pharmacist is responsible for providing medication management, emergency response, clinical support and consultative services for patients located in the Emergency Department, including those awaiting admission to the hospital. The Clinical EM Pharmacist will respond to all code blue situations both in the emergency department and in the hospital. The Clinical EM Pharmacist will complete medication histories of both behavioral and medical patients, boarding in the ED. Hours: 56 hours per pay period (bi-weekly) Schedule: Rotating day/evening, 8-hour shifts, scheduled as “7 shifts on, 7 shifts off." High volumes and acuity create a fast-paced environment for providing pharmaceutical care to critically ill patients in the southern metro of the Twin Cities. Of note, Ridges has also been awarded "Best Hospital Workplace" by the Minnesota Hospital Association in both 2023 and 2024! Responsibilities Job Description Clinical Pharmacists at Ridges: Provide pharmaceutical care to hospital patients. Review new medication orders for appropriateness of therapy Monitor patients on an ongoing basis and adjust medications and doses after consulting with the prescriber Provide interpretation and problem solving of physician’s orders, checking for formulary status, appropriate drug selection, dose and dosage form, drug-drug interactions, drug-nutrient interactions, drug duplication, potential for adverse events, appropriate route of administration, therapeutic substitution, IV to PO conversion, or other formulary or departmental initiatives Evaluate therapy for optimal patient outcomes while ensuring medication safety Perform admission medication histories and discharge medication reconciliation review Coordinate drug distribution in assigned areas. Verify medication orders ordered by providers in the electronic health record (Epic) Direct and oversee the drug distribution activities of pharmacy technicians and interns Participate in team meetings/discussions. Foster an environment of teamwork with other department staff Precept students and/or pharmacy residents on rotation Provide training and education to other staff members Provide drug information and consultation to physicians, nurses, patients, and other healthcare professionals. Act as a primary resource for nursing staff on the proper administration of drugs Provide training and education to others as necessary Remain current with new drug information for patient populations served Participate in medication safety and quality improvement activities Complete medication safety reports when issues are identified Identify additional opportunities to improve patient care and the performance of the pharmacy department Respond to all cardio-pulmonary emergencies as assigned to assist with drug preparation and information on emergency medications intervening as appropriate to ensure optimal patient outcomes. Meet all role-related and age specific competencies. Comply with all initial and ongoing competencies within the entity and department. Complete required learning relevant to the role.
