General Summary
Level II Pharmacists provide prescribed pharmaceuticals and drug information as needed for patient care according to professional standards and practices. The Level II Pharmacist position is a transitional role between the distributive and clinical domains within the pharmacy department. Time spent in each domain is contingent upon the needs of each facility and department. Level II Pharmacists may advance to a Level III Pharmacist position based upon job performance and successful completion of the requirements for advancement established in the Pharmacist Career Ladder Program (PCLP) criteria grids.
Essential Functions:
Maintains the requirements located in the PCLP Mandatory and Optional Criteria Grids for the Level I Pharmacist.Provides oversight of safe, accurate, and timely medication order verification including clarification of any medication orders with discrepancies prior to dispensing.Provides pharmacy management and oversight of: - Integrity and security of medications in all care locations - Investigational drugs - Preparation of injectable chemotherapy - Medication wastage, returns, and recalls - Reconciliation of controlled substance wastage - Compounding of formulations - Automated dispensing cabinets - Medication label components and label qualityProvides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the facility's available protocols which may include: - Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up) - Total parenteral nutrition (TPN) consults - Anticoagulation initiation and follow-up pursuant to a consult - Renal dose adjustments - Intravenous to oral conversions - Additional clinical opportunities may be available based upon departmental needs, clinical experience, and performance, and may include unit-based pharmacist, antimicrobial stewardship, patient care rounds, or pain management teamProvides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.Participates in the training and education of pharmacy technicians, interns, students, and/or pharmacy residents.Provides medication event prevention, monitoring, and reporting with an increased emphasis on patient safety.Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications. Participates in the assessment of technology.Participates in quality, safety, and performance initiatives related to medication use at the organizational and departmental levels. Collaborates on operational system/process research activities.Adheres to WellSpan Health's medication-use policies and standard work. Develops and modifies polices and standard work related to the use of medications within the health system. Participates in the health system's formulary process, ensuring an appropriate formulary system is in place.Common Expectations:
