Pharmacist Inpatient Transitions Care
Description Location: UCHealth UCHlth 2450 Peoria Admin Services, US:CO:Aurora Department: Pharmacy IP FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks), 8am - 4:30pm weekdays, with potential for covering holidays and/or weekends in the future Shift: Days Pay: $50.76 - $76.14 / hour. Pay is dependent on applicant's relevant experience Summary: The Transitions of Care Pharmacist will work collaboratively with healthcare professionals to maximize positive clinical outcomes at admission and discharge. The primary functions include: reviewing medication histories completed by pharmacy technicians at admission, address medication-related barriers prior to discharge (i.e., cost, formulary exclusions, adherence, appropriateness of drug selection, performing discharge medication reconciliation, resolving discrepancies via provider communication, and counseling patients at discharge). A key component of this position requires working with and precepting our longitudinal pharmacy resident led TOC service. Willingness to learn and apply both inpatient and outpatient pharmacy skills is essential for this position. Primary work location is at an office building (2450 S. Peoria, Aurora, CO) and communication with providers and patients is completed on the phone or through instant messaging within the electronic health record. Periodically each month, the pharmacist will work on-site at the University of Colorado Hospital, located on the Anschutz Medical Campus in Aurora, CO. Requirements: + Bachelor's Degree in Pharmacy from an accredited school of Pharmacy. + State licensure as a Pharmacist and any relevant life support certification as determined at position level. Responsibilities: + Review, interpret and evaluate medication regimens to identify, prevent and resolve potential or actual medication-related problems. + Identify and addresses medication-related barriers at the point of transition from hospital to community. + Coordinate medication reconciliation at discharge to improve clinical outcomes and help prevent unnecessary readmissions and adverse medication events. + Collaborate with care teams to develop, implement and evaluate medication management quality improvement efforts. + Oversee and provide feedback on medication histories completed by pharmacy technicians We improve lives.
