CHI St. Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services.  CHI St. Alexius Health manages four CAHs in North Dakota - Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. CHI St. Alexius Health offers a comprehensive line of inpatient and outpatient medical services, including: a Level II Trauma Center, primary and specialty physician clinics, home health and hospice services, durable medical equipment services, a fitness and human performance center and ancillary services throughout western and central North Dakota.

Responsibilities

STATEMENT OF PURPOSE: Collaborate in all phases of pharmaceutical services that includes drug distribution and clinical services, respecting all persons for their own personal worth and dignity while maintaining cost effectiveness

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Pharmacist Responsibilities Monitor drug therapy and communicates findings and recommendations to appropriate professionalsCollect PharmDoc intervention data and submit on an ongoing basisActively participate in renal dosing protocolAccurately assess medication orders for appropriateness (dose, frequency, indication, drug-drug interactions)Actively work to help direct nursing staff as to how to resolve problems/errorsActively address & resolve items in Pharmacist work queue in Pharmacy Order Entry SystemActively participate in registering & following Clozapine patients in IVAXAble to be scheduled to cover an area in addition to your primary responsibility (IS, OR, CP, IP, Clinical)Able to be scheduled for a shift in a pharmacy order entry area.Actively participate in pharmacokinetic monitoringActively participate in monitoring/calculating adult TPNActively participate in monitoring/calculating neonatal/pediatric TPNActively participate in anticoagulant management servicesActively participate in pain managementActively participate in monitoring microbiology culturesActively participate in IV to PO switch programPrepare/check intravenous admixturesDrug Information/ADRs/Med ErrorsEducate/counsel patients and customers about their drug regimen(s) and/or disease preventionProvide  timely drug informationDocument  drug information questions on drug information database as defined per policy Actively participate in ADR reporting processActively participate in medication error reporting processMake recommendations for improving safety in the medication use systemFormulary ManagementReview a class of medicationsComplete at least one DUE project per yearPresent one or more items to Pharmacy & Therapeutics Committee per yearPromote Formulary Management and support Formulary Management decisionsDevelops formulary drug monograph for formulary addition request and makes recommendation for P &TPolicies and Standardized ordersParticipate  in writing, revising, or reviewing two or more departmental/house-wide policies and procedures you are responsible for.Educate appropriate staff about new or revised policy and proceduresFollow current policy and proceduresParticipate in review and revision of physician standardized order setsMaintains Professionalism, Competencies and Leadership Standards by Present and/or helps develop an in-service for the department or organizationServe as a committee/group/Performance Improvement Team (PIT) chairperson or co-chairpersonServe as a student or resident preceptorParticipate  in department education by writing departmental newsletter article as assignedActively support and/or participate in clinical researchStaff developmentSuccessfully complete the defined job specific competencies, including competency for age specific, as assigned. 
Qualifications

Education: Graduate of a College of Pharmacy with a minimum of a Baccalaureate Degree in Pharmacy.

Licensure: Licensed as a pharmacist in the State of North Dakota in good standing.
