Pharmacist - Per Diem

MAIN FUNCTION:

Under the general direction of the Director of Pharmacy, participates in responsibility for complete requisition, control and distribution of all biologicals, pharmaceuticals, pharmaceutical information and applicable systems throughout the institution and its facilities to assure that patients receive appropriate pharmaceutical care.

 

DUTIES AND RESPONSIBILITIES:

 

**        1.       Evaluates physician's orders, consulting with members of the nursing and medical staff when necessary to assure appropriate use of pharmaceuticals.

 

**        2.       Dispenses all medications to individual patients on direct order of physician, utilizing appropriate resources such as computerized systems, automated mechanical systems, software, equipment and supplies to ensure a safe and efficient medication use process.

 

**        3.       Prepares and dispenses all special IV Admixtures, TPN, and chemotherapy doses.  Assures that a suitable environment and proper technique is utilized in preparation, labeling, storage, handling, and administration to ensure the safety of personnel, patients, and visitors while monitoring product sterility and integrity.

 

**        4.       Dispenses approved floor stock medications and controlled substances to specified units throughout the institution and its facilities.  Assures that appropriate records and as required by State and Federal Laws, are maintained.

 

**       5.        Monitors patient medication profiles for appropriateness of drug therapy, consulting with the responsible physician as necessary.

 

            6.       Is responsible for designated pharmaceutical purchasing, inventory, management, distribution and control.

 

           7.       Is responsible for the monthly inspection of designated medication storage areas throughout the institution and its facilities.

           

            8.         Maintains complete records as required by hospital and governmental agencies.

 

**        9.        Assures that pharmaceutical care is in conformance with all applicable laws and regulations and current standards of practice while maintaining consistent documentation of physician consults by pharmacist and impact on patient outcome.

 

 

**        10.     Provides patient specific drug information and accurate and comprehensive information about drugs to other pharmacists, other health professionals and patients as appropriate.

 

**        11.      Directly supervises all job duties and responsibilities of pharmacy technicians.

 

**        12.      Participates in Clinical Rounds to ensure appropriate pharmacotherapy is delivered in a cost effective manner.

 

**        13.     Participates in ADR (Adverse Drug Reaction) reporting process through concurrent identification of suspected ADR's.

 

**        14.      Participates in personal continuing education programs to maintain professional competency.

 

**       15.       Participates in PEP (Professional Experience Program), providing institutional and clinical education for Pharmacy Student Externs.

 

           16.      Is responsible for attending all annual mandatory educational programs as required by position.

 

          17.       Participates in multi disciplinary patient care rounds as assigned.

 

          18.       Provides age appropriate care to neonate, infant, children, adolescent, adult and geriatric patients.

 

           19.      Employee understands and demonstrates the importance of satisfying the needs of the customer/patient by interacting with him/her in a friendly and caring way, being attentive to the customer’s needs, both psychologically and physically, and by taking the initiative to maintain communication with the customer in order to provide a secure and pleasant experience with the Arnot Health.

EDUCATION:                     

New York State Licensed Pharmacist.

 

EXPERIENCE:                   

New York State license eligible.

 

CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:

No CPR requirements.

 

PHYSICAL DEMANDS:    

Some physical effort required.  Light objects lifted, some stooping and reaching.  Required to be on feet much of the time.

 

EXPOSURE CATEGORY:

Category III. Tasks that involve no exposure to blood, body fluids, or tissues.  And Category I tasks are not a condition of employment.
