02170 Pharmacist STRHS-Lawrenceburg Job Description 3.31.2017The Pharmacist is responsible for meeting the needs of internal and external Hospital customers and supporting the department by providing a variety of pharmacy services. This position provides the knowledge and skills to meet physician, patient, staff, and/or hospital needs and performs in accordance with hospital standards and policies. This position is a highly trained individual who interprets physician's orders and compounds and dispenses medications and other pharmaceuticals under critical time constraints and operates under minimal supervision. Coordinates and provides the pharmaceutical care of patients.

Checks for accuracy after filling by technicians.Maintains adequate knowledge of pharmacy automated systems and computer software.Assist with Pharmacy and Therapeutics CommitteeProvides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students.Documents interventions and their outcomes for use in drug therapy quality improvement efforts.Assist with implementation and monitoring of clinical initiativesCoordinate clinical pharmacy workflow, initiatives and goalsBe a role model and resource for staff pharmacistsServe as a liaison to clinical departments and providersAssist with USP 797 and 800 compliance and competencyStaff pharmacist duties
Job Requirements ?Minimum Education
Doctor of Pharmacy (PharmD) with one-year residency requiredRequired Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Required Skills
Certifications:
PGY-1 Residency or BCPSRequired Skills
Licenses: Current and active Pharmacist license in the state of Tennessee required.
